大剂量呋塞米持续静脉泵入治疗老年重度心力衰竭患者的疗效观察

摘　　要：目的探讨重度心力衰竭患者应用大剂量呋塞米持续静脉泵入的治疗效果。方法79例重度心力衰竭患者为研究对象,依据治疗方法不同分为研究组(40例)与对照组(39例)。对照组应用120 mg呋塞米静脉推注,研究组以10 mg/h的速度使用微量泵泵入200 mg呋塞米。对比两组治疗后的心功能指标[心输出量(CO)、左室射血分数(LVEF)、左心室收缩末期内径(LVESD)、左心室舒张末期内径(LVEDD)]及治疗效果。结果研究组CO(4.36±0.17)L/min、LVEF(51.21±4.62)%、LVESD(53.25±2.93)mm、LVEDD(56.91±4.14)mm均优于对照组的(4.11±0.46)L/min、(45.93±5.39)%、(57.32±4.29)mm、(59.42±5.38)mm,差异有统计学意义(P<0.05)。研究组治疗总有效率92.50%高于对照组的74.36%,差异有统计学意义(P<0.05)。结论重度心力衰竭患者应用大剂量呋塞米持续静脉泵入治疗,可有效改善患者心功能,提高治疗效果,值得临床推广应用。Objective To discuss the effect of continuous intravenous pumping of high-dose furosemide in the treatment of elderly patients with severe heart failure.Methods A total of 79 patients with severe heart failure were selected as the research subjects,and they were divided into research group(40 cases)and control group(39 cases)according to different treatment methods.The control group received intravenous bolus injection of 120 mg of furosemide,and the research group received intravenous pumping of 200 mg of furosemide at a rate of 10 mg/h.both groups were compared in terms of cardiac function indexes[cardiac output(CO),left ventricular ejection fraction(LVEF),left ventricular end-systolic diameter(LVESD),left ventricular end-diastolic diameter(LVEDD)],and therapeutic effect after treatment.Results The CO(4.36±0.17)L/min,LVEF(51.21±4.62)%,LVESD(53.25±2.93)mm,and LVEDD(56.91±4.14)mm in the research group were better than(4.11±0.46)L/min,(45.93±5.39)%,(57.32±4.29)mm,and(59.42±5.38)mm in the control group,and the differences were all statistically significant(P<0.05).The total effective rate 92.50%of the research group was higher than 74.36%of the control group,and the difference was statistically significant(P<0.05).Conclusion Continuous intravenous pumping of high-dose furosemide for elderly patients with severe heart failure can effectively improve the cardiac function of patients and improve the therapeutic effect,which is worthy of clinical promotion and application.
