摘 要:目的分析转化生长因子β_(1)(TGF-β_(1))基因多态性与妊娠期糖尿病(GDM)发病风险的关联性。方法选择2019年1月至2020年12月于宝鸡市妇幼保健院产科检查并分娩的孕妇106例,其中GDM组60例,正常组46例,将GDM组孕妇分为A1级25例和A2级35例,应用聚合酶链式反应-限制性片段长度多态性(RFLP-PCR)法检测TGF-β_(1)基因单核苷酸多态位点+869T/C的基因型,分析其基因型频率以及等位基因频率,比较两组孕妇血清TGF-β_(1)水平,比较GDM组TGF-β_(1)基因+869T/C位点三种基因型患者血清TGF-β_(1)水平,并比较不同基因型GDM患者糖脂代谢指标[空腹血糖(FPG)、糖化血红蛋白(HbA1c)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)]水平。结果GDM组孕妇的TGF-β_(1)+869T/C位点TT基因型、T等位基因频率分别为3.33%、20.00%,明显低于正常组的30.43%、52.17%,而CC基因型、C等位基因频率分别为63.33%、80.00%,明显高于正常组的26.09%、47.83%,差异均具有统计学意义(P<0.05);GDM组孕妇的血清TGF-β_(1)水平为(213.68±28.93)ng/mL,明显高于正常组的(165.81±25.36)ng/mL,差异具有统计学意义(P<0.05);A1级与A2级孕妇的血清TGF-β_(1)水平比较差异无统计学意义[(208.97±27.72)ng/mL与(217.04±30.58)ng/mL](P>0.05);GDM组TGF-β_(1)+869T/C位点CC基因型孕妇血清TGF-β_(1)水平为(224.61±31.85)ng/mL,明显高于TT、TC基因型的(152.29±24.47)ng/mL、(199.05±26.21)ng/mL,且TC基因型孕妇血清TGF-β_(1)水平明显高于TT基因型,差异均有统计学意义(P<0.05);CC基因型GDM孕妇的FPG、TG水平分别为(5.15±0.53)mmol/L、(2.59±0.36)mmol/L,明显高于TT+TC基因型的(4.78±0.57)mmol/L、(2.40±0.28)mmol/L,而HDL-C水平为(1.87±0.14)mmol/L,明显低于TT+TC基因型的(1.96±0.17)mmol/L,差异均有统计学意义(P<0.05),但HbA1c、TC及LDL-C水平与TT+TC基因型孕妇比较,差异均无统计学意义(P>0.05)。结论TGF-β_(1)基因+869T/C�Objective To analyze the association between transforming growth factor-β_(1)(TGF-β_(1))gene polymorphism and risk of gestational diabetes mellitus(GDM).Methods A total of 106 pregnant women who were examined and delivered in Department of Obstetrics,Baoji Maternal and Child Health Hospital from January 2019 to December 2020 were selected,including 60 cases in GDM group and 46 cases in normal group.Pregnant women in GDM group were divided into grade A1(n=25)and grade A2(n=35).Polymerase chain reaction-restricted fragment length polymorphism(RFLP-PCR)was used to detect the+869T/C genotype of single nucleotide polymorphism loci of TGF-β_(1) gene,and the genotype frequency and allele frequency were analyzed,and the serum TGF-β_(1) level was compared between the two groups.The level of serum TGF-β_(1) was compared among patients with three genotypes at+869T/C locus of TGF-β1 gene in GDM group,and the levels of glucolipid metabolism indexes[fasting plasma glucose(FPG),glycosylated hemoglobin(HbA1c),total cholesterol(TC),triglyceride(TG),high-density lipoprotein cholesterol(HDL-C),low-density lipoprotein cholesterol(LDL-C)]of GDM patients with different genotypes were compared.Results The TT genotype frequency and T allele frequency of TGF-β_(1)+869T/C locus were 3.33%and 20.00%in GDM group,which were significantly lower than 30.43%and 52.17%in normal group,and the CC genotype frequency and C allele frequency were 63.33%and 80.00%,significantly higher than 26.09%and 47.83%in normal group(P<0.05).The level of serum TGF-β_(1) in GDM group was(213.68±28.93)ng/mL,which was significantly higher than(165.81±25.36)ng/mL in normal group(P<0.05).There was no statistically significant difference in serum TGF-β_(1) level between patients with grade A1 GDM and patients with grade A2 GDM:(208.97±27.72)ng/mL vs(217.04±30.58)ng/mL,P>0.05.The serum TGF-β_(1) level of patients with CC genotype at TGF-β_(1)+869T/C locus in GDM group was(224.61±31.85)ng/mL,which was significantly higher than(152.29±24.47)ng/mL of patient
