自拟清肺消痤汤联合强脉冲光治疗冲任失调型寻常痤疮疗效观察

摘　　要：目的观察自拟清肺消痤汤联合强脉冲光治疗冲任失调型寻常痤疮的临床效果。方法选取2020年1月至2021年10月于西安宝石花长庆医院皮肤科治疗的90例女性寻常痤疮冲任失调型患者为研究对象,依据随机数表法分为对照组与观察组,每组45例。对照组患者使用强脉冲光治疗,观察组患者在对照组治疗的基础上应用自拟清肺消痤汤治疗,两组均治疗4周。治疗后,比较两组患者的临床疗效、治疗前后的性激素指标[黄体生成激素(LH)、雌二醇(E2)、睾酮(T)、卵泡刺激素(FSH)]和炎症指标[肿瘤坏死因子(TNF-α)、白细胞介素-4(IL-4)、白细胞介素-17(IL-17)]水平以及不良反应发生情况。结果观察组患者的治疗总有效率为93.33%,明显高于对照组的77.78%,差异有统计学意义(P<0.05);治疗前,两组患者的LH、E2、T、FSH水平比较差异均无统计学意义(P>0.05);治疗后,观察组患者的LH、FSH水平分别为(78.23±9.16)U/L、(21.69±2.39)U/L,明显高于对照组的(56.23±7.69)U/L、(14.43±1.99)U/L,T水平为(1.85±0.24)nmol/L,明显低于对照组的(2.20±0.31)nmol/L,差异均有统计学意义(P<0.05);治疗前,两组患者的TNF-α、IL-4、IL-17水平比较差异均无统计学意义(P>0.05);治疗后,观察组患者的TNF-α、IL-4、IL-17水平分别为(25.93±3.97)ng/L、(18.85±3.23)ng/L、(0.45±0.12)ng/L,明显低于对照组的(37.27±4.33)ng/L、(26.25±3.11)ng/L、(1.55±0.19)ng/L,差异均有统计学意义(P<0.05);观察组和对照组患者的总不良反应发生率分别为40.00%、42.22%,差异无统计学意义(P>0.05)。结论自拟清肺消痤汤联合强脉冲光治疗冲任失调型寻常痤疮患者效果显著,同时能改善患者性激素水平,值得推广应用。Objective To observe the clinical effect of self-made Qingfei Xiaocuo Decoction combined with intense pulsed light in the treatment of acne vulgaris with Chong-Ren disorder.Methods A total of 90 female patients of acne vulgaris with Chong-Ren disorder who were treated in the Department of Dermatology,Gem Flower Xi'an Changqing Staff Hospital from January 2020 to October 2021 were selected and divided into a control group and an observation group according to the random number table method,with 45 patients in each group.The patients in the control group were treated with intense pulsed light,and the patients in the observation group were treated with self-made Qingfei Xiaocuo Decoction on the basis of the treatment in the control group,both for 4 weeks.After treatment,the clinical efficacy,sex hormone indicators[luteinizing hormone(LH),estradiol(E2),testosterone(T),follicle stimulating hormone(FSH)],inflammatory index[tumor necrosis factor(TNF-α),Interleukin-4(IL-4),interleukin-17(IL-17)]before and after treatment,and the occurrence of adverse reactions were compared between the two groups.Results The total effective rate of treatment in the observation group was 93.33%,which was significantly higher than 77.78%in the control group(P<0.05);before treatment,the LH,E2,T,and FSH levels of the two groups showed no statistically significant difference(P>0.05);after treatment,the levels of LH and FSH in the observation group were(78.23±9.16)U/L and(21.69±2.39)U/L,which were significantly higher than(56.23±2.39)U/L,(14.43±1.99)U/L in the control group,and T level were(1.85±0.24)nmol/L,which was significantly lower than(2.20±0.31)nmol/L of the control group,with statistically significant differences(P<0.05).Before treatment,there was no significant difference in the levels of TNF-α,IL-4,and IL-17 between the two groups(P>0.05);after treatment,the levels of TNF-α,IL-4,IL-17 levels in the observation group were(25.93±3.97)ng/L,(18.85±3.23)ng/L,(0.45±0.12)ng/L,which were significantly lower than(37.27±4.33)ng/L,
