门诊老年患者用药咨询情况分析及改进措施

摘　　要：目的调查分析门诊老年患者用药咨询情况,在实施药学服务过程中加强用药指导,提出改进措施,提高患者依从性。方法选取本科室2020年7月~2021年6月期间用药咨询记录及处方干预记录的399例门诊老年患者,分析老年患者构成状况,用药咨询状况,整理归纳患者满意度。结果399例患者中201例对咨询结果表示满意,143例患者对咨询结果较满意,满意度为86.22%,其余患者均为询问结果不满意。结论经满意度调查得知,我院门诊用药咨询方面还存在不足之处,门诊药师应当重视患者疑虑,实施科学指导,提高门诊用药咨询服务质量,定期整理用药咨询纪录及处方干预记录,掌握其中咨询较多问题并总结经验,提高患者用药安全。Objective To investigate and analyze the medication consultation of elderly outpatients,strengthen medication guidance in the process of implementing pharmaceutical care,and put forward improvement measures to improve patient compliance.Methods 399 elderly outpatients with medication consultation records and prescription intervention records in the undergraduate department from July 2020 to June 2021 were selected to analyze the composition and medication consultation status of elderly patients,sort out and summarize patient satisfaction.Results Among the 399 patients,201 were satisfied with the consultation results and 143 were partially satisfied,with a satisfaction rate of 86.22%.The rest patients were not satisfied with the results.Conclusion The satisfaction survey shows that there are still deficiencies in outpatient medication consultation in our hospital.Outpatient pharmacists should pay attention to patients'doubts,implement scientific guidance,improve the quality of outpatient medication consultation service,regularly sort out medication consultation records and prescription intervention records,master more consultation problems and summarize experience,and improve patients'medication safety.
