基于RCT的血栓通胶囊治疗瘀血阻滞证冠心病心绞痛药物经济学评价研究

摘　　要：目的观察血栓通胶囊治疗瘀血阻滞证冠心病心绞痛的药物经济学效果,通过药物经济学评价,为政府和相关部门制定治疗瘀血阻滞证冠心病心绞痛相关政策提供科学依据与参考。方法入选2018年11月~2019年12月在黑龙江中医药大学附属第二医院等6家医院就诊的瘀血阻滞证冠心病心绞痛患者223例为受试者,按照治疗方案分为对照组(55例)和观察组(168例)。治疗组口服血栓通胶囊(2粒/次,3次/天),对照组口服银杏叶胶囊(2粒/次,3次/天);两组疗程均为4周。观察两组的临床疗效,比较两组治疗效果、效用指标、成本、成本-效果、成本-效用的差异。结果观察组中医证候疗效优于对照组(P<0.05);观察组与对照组效用(QALYs)的差异无统计学意义(P>0.05);观察组心绞痛疗效、心绞痛疗效(积分)、中医证候总有效率、中医证候疗效和DUKE评分疗效上的成本效果比和成本效用优于对照组(均P<0.05)。结论血栓通胶囊治疗瘀血阻滞证冠心病心绞痛成本较低,同时效果和效用高,血栓通胶囊与银杏叶胶囊相比具有一定的药物经济性优势。Objective To observe the pharmacoeconomic effects of Xueshuantong Capsules on angina pectoris in coronary heart disease due to blood stasis syndrome through the pharmacoeconomic evaluation of Xueshuantong Capsules,providing a scientific basis and reference for the government and related departments to formulate relevant policies on the treatment of angina pectoris in coronary heart disease due to blood stasis syndrome.Methods 223 patients with angina pectoris in coronary heart disease due to blood stasis syndrome treated in 6 hospitals including the Second Affiliated Hospital of Heilongjiang Chinese Medicine University from November 2018 to December 2019 were selected as the research subjects and randomly divided into the control group(55 patients)and the treatment group(168 patients).The control group took Ginkgo Leaf Capsules orally,2 capsules/time,tid.In the treatment group,Xueshuantong Capsules were taken orally,2 capsules/time,tid.Both groups were treated for 4 weeks.We observed the clinical efficacy of the two groups and compared the treatment effect analysis,utility index,cost analysis,cost-effectiveness evaluation,and cost-utility evaluation of the two groups.Results After 4 weeks of treatment of angina pectoris in coronary heart disease due to blood stasis syndrome,the curative effect of traditional Chinese medicine syndrome(TCM)in Xuantong Capsules group was better than that in Ginkgo Leaf Capsules group(P<0.05),and the difference between two groups was statistically significant;there was no significant difference between the efficacy of Xueshuantong Capsules and Ginkgo Leaf Capsules(QALYs)(P>0.05);the effect of treating angina and treating angina(score),total effective rate of TCM syndrome,TCM syndrome efficacy,cost-effectiveness ratio and cost-utility ratio of DUKE score efficacy in Xueshuantong Capsules group were better than Ginkgo Leaf Capsules group(P<0.05),the difference was statistically significant.Conclusion Xueshuantong Capsule is less costly in treating angina pectoris of coronary heart dise
