新型睾丸固定垫在睾丸穿刺术后的应用

摘　　要：目的评估新型睾丸固定垫在睾丸穿刺中的应用效果。方法87例需要施行睾丸取材诊断或取精用于单精子显微胞浆内注射(ICSI)辅助生育患者,其中58例选用常规消毒纱布给予术后固定止血为A组,29例采用新型睾丸固定垫进行术后固定止血为B组。比较两组患者的年龄、睾丸体积、平均手术时间、手术期间的疼痛评分及术后愈合天数,患者满意度和术后并发症情况。结果A组与B组患者术后愈合天数分别为5~14(9±3.81)d和4~9(6±2.01)d(P<0.05),患者满意度分别为52%(30/58)和93%(27/29)(P<0.05),差异有显著性(P<0.05),术后并发症情况,A组术后并发症发生率为10.3%(6例),其中,阴囊红肿4例,睾丸肿痛1例,睾丸硬结1例:B组术后并发症发生率为3%,为1例,阴囊轻度红肿,差异有显著性(P<0.05),而两组的患者的年龄(28±5.6)岁和(29±6.1)岁、睾丸体积(15±4.9)mL和(16±4.3)mL、平均手术时间(8±3.7)min和(8±4.1)min、手术期间的疼痛评分(2.74±0.75)和(2.84±0.76),比较差异无显著性。结论新型睾丸固定垫固定具有止血效果好、并发症少及有利于术后愈合等特点,值得临床推广运用。Objective To evaluate the clinical effects of a new testicular Fixing Pad(TFP)after the testicular puncture.Methods Total of 87 patients who need testicular puncture for diagnosis or ICSI were recruited in the study.After testicular puncture,58 of them were treated with usual Sterile gauze(group A,n=58),and other 29 with TFP(group B,n=29).Age,testis volume,average operation time,pain scores during-operation,healing time,incidence rates of complication and degree of satisfaction were comparatively analyzed between two groups.Results The degree of satisfaction in the group A(52%)was remarkably lower than that in the group B(93%).The healing time in group B 4~9d(6±2.01)was significantly shorter than that in the group A 5~14d(9±3.81)d(P<0.05).The rate of postoperative complications in group A(10.3%)was obviously higher than that in group B(3%).But there were no significant differences between the two groups in the patients’age(28±5.6)vs(29±6.1)year,testicular volume(15±4.9)vs(16±4.3)mL,the average operation time(8±3.7)vs(8±4.1)min,pain scores during-operation(2.74±0.75)vs(2.84±0.76)(P>0.05).Conclusion New testicular Fixing Pad(TFP)shows some advantages in hemostatic effect,less complications and postoperative healing,which is worth popularizing and applying in clinic.
