药物联合清宫治疗剖宫产术后子宫瘢痕妊娠效果评价

摘　　要：目的探讨剖宫产术后子宫瘢痕妊娠清宫治疗的效果。方法选取医院2021年1月至12月收治的剖宫产术后子宫瘢痕妊娠患者84例,随机分为E组和F组,各42例。E组患者采用甲氨蝶呤+米非司酮治疗,F组患者在E组基础上增加清宫治疗,对比两组临床疗效、妊娠包块、β-人绒毛膜促性腺激素(β-HCG)水平及恢复效果。结果F组的治疗总有效率为95.24%,显著高于E组的80.95%(P<0.05);两组治疗后,F组妊娠包块缩小效果优于E组,且F组β-HCG水平显著低于E组(P<0.05);F组妊娠包块消失时间为(28.07±3.29)d、血清β-HCG恢复正常时间为(26.87±3.48)d、月经复潮时间为(43.59±6.53)d,均短于E组的(39.88±3.56)d、(37.46±3.95)d、(53.72±6.84)d(P<0.05);F组患者对治疗的认可度为92.86%,显著高于E组的76.19%(P<0.05)。结论剖宫产术后子宫瘢痕妊娠患者应用甲氨蝶呤+米非司酮+清宫治疗,相较于单一药物治疗的效果更显著,可促进患者早日恢复,患者认可度较高。Objective To investigate the effect of curettage in the Treatment of uterine scar pregnancy after cesarean section.Methods A total of 84 patients with uterine scar pregnancy after cesarean section treated in the hospital from January to December 2021were selected and randomly divided in to the group E and the group F,with 42 cases in each group.The patients in the group E were given the treatment of methotrexate+mifepristone,on this basis,the patients in the group F were given curettage treatment.The clinical efficacy,pregnancy mass,the level ofβ-human chorionic gonadotropin(HCG)and recovery effect between the two groups were compared.Results The total effective rate of group F was 95.24%,which was significantly higher than 80.95%of the group E(P<0.05).After treatment,the effect of pregnancy mass reduction in the group F was better than that in the group E,while the level ofβ-HCG in the group F was significantly lower than that in the group E(P<0.05).The(28.07±3.29)d of disappearance time of pregnancy,the(26.87±3.48)d of recovery time of serumβ-hCG and(43.59±6.53)d of menstruation recovery time in group F were significantly shorter than(39.88±3.56)d,(37.46±3.95)d and(53.72±6.84)d in the group E(P<0.05).The treatment acceptance in the group F was 92.86%,which was significantly higher than 76.19%of the group E(P<0.05).Conclusion Compared with single drug therapy,the effect of methotrexate,mifepristone combined with curettage treatment is more significant,which can promote the early recovery of patients and it is accepted by them.
