摘　　要：目的探讨糖尿病周围神经病变(DPN)患者外周血辅助性T细胞17(Th17)的表达特征以及临床应用。方法选取上海市杨浦区精神卫生中心和复旦大学附属中山医院徐汇医院接诊DPN患者34例作为病例组,另在同期人群中选取单纯糖尿病患者30例作为对照1组,同期健康体检人群中选取健康群体30例作为对照2组。对入组对象静脉血采集,经处理后采用流式细胞仪测定Th17细胞亚群,全自动生化分析仪测定糖化血红蛋白、空腹血糖,行组间对比、相关性分析以及受试者工作特征(ROC)曲线分析。结果病例组Th17、糖化血红蛋白、空腹血糖检测结果均高于对照1组、对照2组,差异有统计学意义(P<0.05);Spearman相关分析,Th17与糖化血红蛋白、空腹血糖呈正相关性(r=0.536、0.348,P<0.05);ROC曲线计算,AUC=0.821,灵敏度为73.8%,特异度为83.9%。结论DPN患者呈现为明显的外周血Th17高表达,且具有较高的预测价值。Objective To investigate the expression and clinical application of peripheral blood helper T cell 17(Th17)in patients with diabetic peripheral neuropathy(DPN).Methods Thirty four patients with DPN in Shanghai Yangpu District Mental Health Center and Shanghai Xuhui Central Hospital were selected as the case group.Thirty cases of simple diabetes mellitus were selected as the reference group in the same period.The 1 groups were selected as the reference group.Thirty healthy people were selected as the reference group in the same period.The 2 groups were selected as the reference group.After treatment,Th17 cell subsets were measured by flow cytometry,glycosylated hemoglobin and fasting blood glucose were measured by automatic biochemical analyzer,and inter group comparison,correlation analysis and ROC curve analysis were carried out.Results The results of Th17,glycosylated hemoglobin and fasting blood glucose in the case group were higher than those in reference group 1 and reference group 2,the difference was statistically significant(P<0.05);Spearman correlation analysis showed that Th17 was positively correlated with glycosylated hemoglobin and fasting blood glucose(r=0.536,0.348,P<0.05);ROC curve calculation,AUC=0.821,sensitivity 73.8%,specificity 83.9%.Conclusion Patients with DPN have obvious high expression of Th17 in peripheral blood and have high predictive value.
