摘　　要：目的:探讨关节镜下改良Brostrom-Mason-Allen术治疗复发性踝关节外侧副韧带损伤的临床疗效。方法:回顾性分析我院2018年1月~2020年1月接受关节镜下改良Brostrom-Mason-Allen术治疗的30例复发性踝关节外侧副韧带损伤患者,男18例、女12例,左侧13例、右侧17例。通过比较患者术前及术后1周、3月及末次随访时疼痛视觉模拟评分(VAS),术前及术后2月、6月、1年及末次随访时美国足踝外科协会(AOFAS)评分以及末次随访使用关节韧带数字体查仪(Ligs)测量患者双侧踝关节前抽屉实验位移以评价手术疗效。结果:术后患者切口均一期愈合,未出现感染、植入反应、肌腱损伤、神经损伤或血管损伤等并发症。患者随访时间24.3个月(20~36个月)。患者术后VAS评分、AOFAS评分较术前明显改善,且差异具有统计学意义(P<0.001)。患者末次随访时踝关节前抽屉实验健侧与患侧位移(11.34±1.35 mm vs. 11.36±1.18 mm),差异无统计学意义(t=-0.08,P>0.05)。结论:关节镜下改良Brostrom-Mason-Allen术治疗复发性踝关节外侧副韧带损伤,手术创伤小,近期临床疗效好且安全可靠,术后踝关节功能恢复满意。Objective To explore the effect of the arthroscopic modified Brostrom-Mason-Allen procedure treating recurrent lateral ankle ligament injuries. Methods A totally of 30 patients with a recurrent lateral ankle ligament injury(including 18 males and 12 females,13 left and 17 right) and undergoing the arthroscopic modified Brostrom-Mason-Allen procedure between January 2018 and January2020 were selected. They were evaluated pain using the visual analogue score(VAS) preoperatively,as well as one week and three months postoperatively and during the last follow-up. Meanwhile, their American Orthopedic Foot and Ankle Society(AOFAS) scores were recorded before as well as 2,6and 12 months after the operation,and during the last follow-up. Moreover,the displacement of the bilateral anterior ankle drawer test was measured at the last follow-up with a digital physical examination device(Ligs). Results All patients had one-stage postoperative healing of the incision without complications such as infections,implantation reactions,as well as tendon,nerve or vascular injury. All patients were followed up for an average of 24.3 months(ranging from 20 to 36 months). The postoperative VAS and AOFAS scores improved significantly compared with those before the operation(P<0.001). However,no significant differences were found in the displacement of the anterior ankle drawer test at the final follow-up between the healthy and the affected sides(11.34 ± 1.35 mm vs. 11.36± 1.18 mm)(t=-0.08,P>0.05). Conclusion The arthroscopic modified Brostrom-Mason-Allen procedure is a minimally invasive procedure with excellent and safe clinical outcomes in the short term for patients with the recurrent lateral collateral ligament injury of the ankle,and the postoperative functional recovery of the ankle joint is satisfactory.
