摘 要:目的:采用高速双平面荧光透视成像系统(DFIS)探究着鞋与裸足条件下跑步时整个支撑阶段第1跖趾关节在体六自由度(6DOF)运动学差异。方法:招募15名健康男性跑者,通过计算机断层扫描技术重建第1跖趾关节三维骨骼模型,并建立局部坐标系。利用DFIS采集受试者着鞋与裸足下后跟着地时(跑速:3±5%m/s)的荧光成像,通过2D-3D配准技术获取第1跖趾关节支撑期6DOF运动学特征,并采用配对样本t检验比较。结果:相比裸足,着鞋1)降低了支撑期第1跖趾关节峰值内移、后移和上移活动(P<0.05);2)降低了最小、最大伸展角度和屈伸活动范围(P<0.05);3)增加了第1跖趾关节最小内收角度(P<0.05)。结论:着鞋限制了跑步支撑期第1跖趾关节屈伸运动并增加了水平面内收角度,提示着鞋可能会影响其蹬伸效果并增加拇外翻发生的风险。Objective To observe the difference in the 6 degree of freedom(6DOF) kinematics of the first metatarsophalangeal joint(MPJ)in the supporting phase during shod and barefoot running using a high-speed dual fluoroscopic imaging system(DFIS). Methods Fifteen healthy male runners were recruited. Computed tomography scans were taken of each participant’s 1stMPJ of the right foot for the construction of 3D models and local coordinate system. Then their fluoroscopic images when rear feet struck the track at a speed of 3 ± 5% m/s were captured,and the 6DOF kinematics was calculated using 2D-3D registration. Paired sample t-test was conducted to compare the 6DOF kinematic characteristics of the 1stMPJ in the stance phase during shoe-wearing and barefoot running. Results Compared with barefoot,wearing shoes decreased the peak medial,posterior,and superior translation of the 1stMPJ in the stance phase(P<0.05). Meanwhile,the maximum extension angle,minimum extension angle,and flexion/extension range of motion of the 1stMPJ were decreased,and the minimum adduction angle was increased(P<0.05). Conclusion Shoe-wearing limits the flexion and extension,and increases the adduction angle of the 1stMPJ in the stance phase during running,suggesting that shoes may affect the push-off of the 1stMPJ and increase the risk of hallux valgus.
