摘　　要：目的:测算海南省心血管疾病患者的经济负担并分析其影响因素,为政策制定提供参考依据。方法:利用海南省2019年卫生总费用核算数据,计算心血管疾病经济负担;利用多元回归模型对患者门诊及住院费用的影响因素进行分析。结果:2019年海南省心血管疾病的直接经济负担7.22亿元,间接经济负担1.93亿元;药占比、机构级别、有无医保对门诊费用存在显著影响,药占比、是否手术、住院天数及机构级别对住院费用存在显著影响。结论:持续优化医疗处方行为,降低药占比;加强老年人心脑血管疾病的预防与控制;合理减少住院时间,推进分级诊疗;建立完善统一的疾病负担评价指标库,降低患者整体疾病经济负担。Objective To measure the economic burden of cardiovascular disease patients in Hainan Province and analyze its influencing factors, so as to provide a reference basis for policy formulation. Methods The economic burden of cardiovascular diseases was calculated using the 2019 total health cost accounting data of Hainan Province. The influencing factors of patients’ outpatient and inpatient costs were analyzed using multiple regression models. Results The direct economic burden of cardiovascular diseases in Hainan Province in 2019was RMB 722 million, and the indirect economic burden was RMB 193 million. There were significant effects of drug occupancy ratio, institution level and type of medical insurance on outpatient costs, and significant effects of whether surgery was performed, days of hospitalization and institution level on inpatient costs. Conclusion It is suggested to continuously optimize medical prescription behaviors to reduce the drug occupancy ratio, strengthen the prevention and control of cardiovascular and cerebrovascular diseases in the elderly, reasonably reduce the length of hospitalization and promote graded treatment, and establish and improve a unified database of disease burden evaluation indicators to reduce the overall economic burden of disease on patients..
