摘　　要：目的探究老年糖尿病酮症酸中毒(DKA)伴感染患者人分泌型磷脂酶A2-X(sPLA2-X)、β-防御素3(hβD-3)、C-反应蛋白与pH比值(CRP/pH)水平变化及临床意义。方法选取本院117例老年DKA患者,分为感染组(n=21)、DKA组(n=96),比较2组基线资料、sPLA2-X、hβD-3、CRP/pH,分析各指标临床应用价值。结果感染组呼吸系统慢性疾病患者占比、sPLA2-X、CRP/pH、APACHEⅡ评分均高于DKA组,hβD-3低于DKA组(P<0.05);sPLA2-X、CRP/pH与APACHEⅡ评分间存在正相关关系,hβD-3与APACHEⅡ评分间存在负相关关系(P<0.05);sPLA2-X、CRP/pH高水平者生存率低于低水平者,hβD-3高水平者生存率高于低水平者(P<0.05)。结论老年DKA伴感染患者sPLA2-X、CRP/pH水平明显升高,hβD-3水平异常降低,检测各指标水平有助于临床评估DKA伴感染患者病情程度及预后转归方向。Objective To investigate the level changes and clinical significance of human secretory phospholipase A2-X(sPLA2-X),β-defensin 3(hβD-3)and the ratio of C-reactive protein to pH(CRP/pH)in elderly patients with diabetic ketoacidosis(DKA)complicated with infection.Methods A total of 117 elderly patients with DKA in our hospital were selected and divided into infection group(n=21)and DKA group(n=96).The baseline data,sPLA2-X,hβD-3,CRP/pH of the two groups were compared,and the clinical application value of each index was analyzed.Results The proportion of patients with chronic respiratory diseases,sPLA2-X,CRP/pH,and APACHEⅡscores in the infection group were higher than those in the DKA group,but hβD-3 was lower than that in the DKA group(P<0.05).There was a positive correlation between APACHEⅡscores and a negative correlation between hβD-3 and APACHEⅡscores(P<0.05);the survival rate of patients with high levels of sPLA2-X and CRP/pH was lower than those with low levels,while the survival rate of the patients with high hβD-3 level was higher than that of the low-level patients(P<0.05).Conclusion The levels of sPLA2-X and CRP/pH in elderly patients with DKA and infection were significantly increased,but the level of hβD-3 was abnormally decreased.The detection of each index level is helpful for clinical evaluation of the degree of disease and the trend of prognosis in patients with DKA and infection.
