摘　　要：目的建立适合桐乡地区表观健康成人的动脉血钾参考区间,并进行临床适宜性评价。方法选取2021年1月—12月在本院体检并符合筛选标准的120例表观健康人群,按标准规程进行动脉血采集与血钾检测,采用非参数法计算参考区间,以P2.5~P97.5表示。选取同时进行动静脉血钾检测的急诊患者420例行新建立动脉血钾参考区间的适应性与临床效果评价。结果120例表观健康成人动脉血钾检测结果呈偏态分布(Z=1.615,P=0.011)。青年、中年和老年组3年龄组间及各年龄组男、女性别间动脉血钾差异均无统计学意义(P>0.05),非参数统计方法得出的成人动脉血钾的参考区间为3.15 mmol/L~4.76 mmol/L;以参考区间为判断标准,基于动脉血钾和静脉血钾诊断的低钾血症和高钾血症差异均无统计学意义(P>0.05);基于建立的动脉血钾参考区间评估血钾平衡紊乱时,对高、低钾血症具有较高的诊断效能。结论本研究建立了桐乡地区表观健康成人动脉血钾参考区间,并适用于桐乡地区表观健康成年人群。Objective To establish a reference range of arterial blood potassium suitable for apparent healthy adults in Tongxiang area,and to evaluate its clinical suitability.Methods A total of 120 apparently healthy people who had physical examinations in our hospital from January to December 2021 and met the screening criteria were selected to perform arterial blood collection and blood potassium detection according to the standard procedures.The reference interval calculated by the nonparametric method was expressed by P2.5-P97.5.A total of 420 emergency patients were selected to evaluate the adaptability and clinical effect of the newly established reference interval of arterial blood potassium.Results The distribution of arterial blood potassium in 120 normal adults was skewed(Z=1.615,P=0.011).There was no statistical significance on the difference in arterial blood potassium between the three age groups of the youth,the middle age and the old age,and also between male and female in each age group(P>0.05).The reference interval of adult arterial blood potassium obtained by the nonparametric statistical method was 3.15 mmol/L-4.76 mmol/L;there was no statistical significance on the difference in the diagnosis of hypokalemia and hyperkalemia based on arterial and venous blood potassium using the reference interval as the judgment criterion(P>0.05).The established reference interval of arterial blood potassium has high diagnostic efficiency for hyperkalemia and hypokalemia.Conclusion The reference interval of arterial blood potassium in apparently healthy adults in Tongxiang area was established and applied to apparently healthy adults.
