系统免疫炎症指数在类风湿性关节炎患者中的临床应用价值

摘　　要：目的探讨系统性免疫炎症指数(SII)在类风湿性关节炎(RA)患者的诊断和疗效评估中的应用价值。方法收集2021年1月—12月本院风湿免疫科就诊的RA患者144例,干燥综合征患者101例,结缔组织病患者72例,在治疗前空腹采取患者静脉血,分别做血常规、血沉检测,并根据血常规结果计算SII、NLR、LMR等。结果类风湿性关节炎组与对照组相比中性粒细胞、单核细胞、血小板、血沉、NLR、SII升高,淋巴细胞、LMR下降;类风湿性关节炎组与干燥综合征组、结缔组织病组相比,白细胞、中性粒细胞、血小板、血沉、NLR、NMR、SII升高,淋巴细胞、LMR下降;类风湿性关节炎组与干燥综合征组相比单核细胞下降;ROC曲线分析显示SII曲线下面积为0.779。结论SII与RA存在显著相关性,可以作为评估SA的生物学指标,与血常规、NLR等指标的联合应用,对SA的诊断及疗效评估有较好的应用价值。Objective To explore the application value of SII in the diagnosis and efficacy evaluation of RA patients.Methods A total of 144 patients with RA,101 patients with Sjogren’s syndrome and 72 patients with connective tissue disease were collected from the Rheumatology and Immunology Department of our hospital from January 2021 to December.The venous blood was taken on an empty stomach before treatment,and the blood routine and ESR were tested respectively.The blood routine results were calculated,such as SII,NLR,LMR,etc.Results Compared with the control group,neutrophils,monocytes,platelets,ESR,NLR and SII in rheumatoid arthritis group increased,while lymphocytes and LMR decreased;compared with Sjogren’s syndrome group and connective tissue disease group,leukocytes,neutrophils,platelets,ESR,NLR,NMR and SII in rheumatoid arthritis group increased,while lymphocytes and LMR decreased;compared with Sjogren’s syndrome group,monocytes in rheumatoid arthritis group decreased;ROC curve analysis shows that the area under the SII curve is 0.779.Conclusion There is a significant correlation between SII and RA.SII can be used as a biological index to evaluate RA.It can be used in combination with blood routine examination and NLR.It has good application value for the diagnosis and curative effect evaluation of RA.
