摘  要:目的探讨湖州地区人群中细胞色素P4502C19(CYP2C19)基因多态性分布,为临床合理用药提供指导。方法采用荧光PCR技术检测湖州地区拟进行氯吡格雷用药患者的CYP2C19基因多态性,探讨其基因型和代谢型在人群中的分布,并分析与不同地区的差异。结果在688例患者中等位基因分布频率分别为CYP2C19*1为61.48%,CYP2C19*2为32.19%,CYP2C19*3为5.60%,CYP2C19*17为0.73%。患者代谢型主要为中间代谢型(46.80%),其次为快代谢型(37.79%)、慢代谢型(14.39%)、超快代谢型(1.02%)。不同性别患者间基因多态性差异无统计学意义(P>0.05);湖州地区与国内大多地区汉族人群基因型和代谢型分布差异无统计学意义(P>0.05),而与省内杭州地区基因型分布和代谢型分布差异均有统计学意义(P<0.05)。结论湖州地区人群CYP2C19基因型以杂合型为主,代谢型以中间代谢型为主,但慢代谢型也有相当比例,建议用药前进行基因多态性检测指导个体化用药。Objective To investigate the distribution of cytochrome P4502C19(CYP2C19)gene polymorphism in Huzhou area,and to provide guidance for clinical rational drug use.Methods Fluorescent polymerase chain reaction PCR was used to detect the gene polymorphism of patients before treating with clopidogrel in Huzhou area,to explore the distribution characteristics of genotypes and metabolism in the Huzhou population,and to analyze its differences with different regions in China.Results Among the 688 tested patients,the distribution frequencies of genes were 61.48%for CYP2C19*1,32.19%for CYP2C19*2,5.60%for CYP2C19*3,and 0.73%for CYP2C19*17,respectively.The proportion of patients with different metabolism phenotypes was intermediate metabolism type(46.80%),followed by fast metabolism type(37.79%),then slow metabolism type(14.39%),ultra-fast metabolism type(1.02%).There was no statistical significance on the difference in gene polymorphism between different genders(P>0.05).There is no statistical significance on the difference in the distribution of genotypes and metabolic types in Han Population between Huzhou area and other areas of China(P>0.05),but there is statistical significance on the differences in the distribution of genotypes and metabolic types between Huzhou and Hangzhou(P<0.05).Conclusion CYP2C19 genotypes are mainly heterozygous,metabolic types are mainly intermediate metabolic types,but a considerable proportion of slow metabolic types are also found in Huzhou population.It is recommended to conduct genetic polymorphism testing before medication to guide individualized medication.
