摘　　要：目的研究术前预存式自体输血联合术中回收式自体输血对人工全髋关节置换术患者血液流变学和凝血功能的影响。方法选取2020年5月-2021年5月我院治疗52例人工全髋关节置换术患者为研究对象,采用随机数字表法分为对照组和观察组,各26例。对照组采用异体输血,观察组采用术前预存式自体输血联合术中回收式自体输血,比较两组血常规指标、凝血功能指标、血液流变学指标、细胞免疫功能指标以及并发症发生率。结果观察组输血6、24 h后Hb、HCT、PLT、FIB、PT、TT比较,差异无统计学意义(P>0.05);观察组输血6、24 h后血浆黏度、全血黏度高切低于对照组(P<0.05);观察组输血6、24 h后CD3^(+)、CD4^(+)高于对照组(P<0.05);两组并发症发生率比较,差异无统计学意义(P>0.05)。结论术前预存式自体输血联合术中回收式自体输血对人工全髋关节置换术凝血功能、血常规无影响,但会改善患者血液流变学指标,降低免疫功能抑制。Objective To study the effect of preoperative predeposited autologous blood transfusion combined with intraoperative salvaged autologous blood transfusion on hemorheology and coagulation function in patients undergoing total hip arthroplasty.Methods A total of 52 patients with total hip arthroplasty treated in our hospital from May 2020 to May 2021 were selected as the research objects.They were divided into control group and observation group by random number table method,with 26 cases in each group.The control group was treated with allogeneic blood transfusion,and the observation group was treated with preoperative predeposit autologous blood transfusion combined with intraoperative salvaged autologous blood transfusion.The blood routine index,coagulation function index,hemorheology index,cellular immune function index and complication rate were compared between the two groups.Results There was no significant difference in Hb,HCT,PLT,FIB,PT and TT at 6 and 24 h after blood transfusion in the observation group(P>0.05).After 6 and 24 hours of blood transfusion,the plasma viscosity and whole blood viscosity of the observation group were lower than those of the control group,and CD3^(+)and CD4^(+)were higher than those of the control group(P<0.05).There was no significant difference in the incidence of complications between the two groups(P>0.05).Conclusion Preoperative predeposited autologous blood transfusion combined with intraoperative salvaged autologous blood transfusion has no effect on coagulation function and blood routine in total hip arthroplasty,but it can improve the hemorheological indexes and reduce the inhibition of immune function.
