摘 要:目的:分析后路减压植骨联合椎弓根螺钉内固定术运用于胸腰段脊柱骨折合并脊髓损伤(SCI)治疗中的价值。方法:回顾性分析绵阳市中心医院近年来收治的胸腰段脊柱骨折合并SCI患者60例的临床资料。依据不同手术方式分成两组,将其中单纯行椎弓根螺钉内固定术的28例患者纳入常模组,将其中行后路减压植骨联合椎弓根螺钉内固定术的32例患者纳入试验组。比较两组患者术后的伤椎前后缘高度比、Cobb’s角、触觉及运动觉评分。结果:术后6个月,相较于常模组,试验组的伤椎前缘高度比、后缘高度比均更高,Cobb’s角更小(P<0.05)。术后6个月,相较于常模组,试验组的触觉评分、运动觉评分均更高(P<0.05)。结论:胸腰段脊柱骨折合并SCI患者行后路减压植骨联合椎弓根螺钉内固定术的效果显著,可恢复伤椎前后缘高度,改善Cobb’s角,促进触觉及运动觉的恢复。Objective:To analyze the value of posterior decompression and bone grafting combined with pedicle screw internal fixation in the treatment of thoracolumbar spinal fractures complicated with spinal cord injury(SCI).Methods:The clinical data of 60 patients with thoracolumbar spinal fractures complicated with SCI admitted to Mianyang Central Hospital in recent years were retrospectively analyzed.The patients were divided into two groups according to different surgical methods.Twenty-eight patients who underwent pedicle screw internal fixation were included in the normal group,and 32 patients who underwent posterior decompression and bone grafting combined with pedicle screw internal fixation were included in the experimental group.The height ratio of the anterior and posterior edges of the injured vertebra,Cobb’s Angle,touch and motion perception scores were compared between the two groups.Results:At 6 months after operation,compared with the normal group,the anterior height ratio and posterior height ratio of injured vertebrae in the experimental group were higher,and Cobb’s Angle was smaller(P<0.05).At 6 months after operation,the touch score and motion perception score of the experimental group were higher than those of the normal group(P<0.05).Conclusion:Posterior decompression and bone grafting combined with pedicle screw internal fixation is effective in patients with thoracolumbar spinal fractures complicated with SCI,which can restore the height of the anterior and posterior edges of the injured vertebrae,improve Cobb’s Angle,and promote the recovery of touch and motion sensation.
