摘　　要：目的:研究改良经脐单孔腹腔镜对子宫肌瘤剔除术患者围术期指标、术后并发症和切口美观满意度的影响。方法:选择2020年7月至2021年12月在河池市人民医院接受治疗的子宫肌瘤患者150例,按随机数表法分为A组(n=50)、B组(n=50)、C组(n=50)。A组予以改良经脐单孔腹腔镜手术治疗,B组予以传统经脐单孔腹腔镜手术治疗,C组予以传统3~4孔腹腔镜手术治疗。比较三组患者的围术期指标、术后并发症发生率及切口美观满意度。结果:三组患者的术后血红蛋白下降幅度、术后24h疼痛评分、术后住院时间相比,差异无统计学意义(P>0.05);三组患者术后均无并发症发生;A组的手术时间、术中出血量少于B组、C组,差异有统计学意义(P<0.05);A组的术后切口美观满意度评分高于B组、C组,差异有统计学意义(P<0.05)。结论:改良经脐单孔腹腔镜手术对改善子宫肌瘤剔除术患者的围术期指标具有积极作用,患者术后切口美观满意度显著提升,术后无并发症发生,具有积极推广价值。Objective:To investigate the effect of modified transumbilical single-port laparoscopy on perioperative indicators,postoperative complications and incision aesthetic satisfaction in patients undergoing myomectomy.Methods:A total of 150 patients with uterine fibroids treated in Hechi People’s Hospital from July 2020 to December 2021 were selected and divided into group A(n=50),group B(n=50),and group C(n=50)according to the random number table method.Group A was treated with modified transumbilical single-port laparoscopic surgery,group B was treated with traditional transumbilical single-port laparoscopic surgery,and group C was treated with traditional 3-4-port laparoscopic surgery.The perioperative indicators,postoperative complication rate and incision aesthetic satisfaction were compared among the three groups.Results:There were no significant differences in postoperative hemoglobin decline,postoperative pain score at 24 hours,and postoperative hospital stay among the three groups(P>0.05).No postoperative complications occurred in the three groups.The operation time and intraoperative blood loss in group A were less than those in group B and C,and the differences were statistically significant(P<0.05).The postoperative incision aesthetic satisfaction score of group A was higher than that of groups B and C,and the difference was statistically significant(P<0.05).Conclusion:Modified transumbilical single-port laparoscopic surgery has a positive effect on improving the perioperative indicators of patients undergoing myomectomy.The postoperative incision aesthetic satisfaction of patients is significantly improved,and there are no postoperative complications,which is of positive promotion value.
