中药贴敷治疗哺乳期急性乳腺炎的疗效观察

摘　　要：目的:观察对哺乳期急性乳腺炎患者进行中药贴敷治疗的临床效果。方法:本临床试验采取前瞻性随机对照研究的方法,将2020年12月1日至2021年7月31日在乌鲁木齐友爱医院乳腺科住院治疗的符合哺乳期急性乳腺炎(急性单纯炎症期)诊断标准且中医辨证属胃热壅滞型的120例患者随机(采用随机数表法)分为两组,治疗组和对照组各60例。治疗组采用头孢呋辛钠静滴+中药贴敷+手法按摩通乳治疗,对照组采用头孢呋辛钠静滴+手法按摩通乳治疗。比较两组的疗效及各项临床指标。结果:治疗后第2天及第3天,治疗组的乳腺炎症范围均小于对照组,血清白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)、超敏C反应蛋白(hs-CRP)水平均低于对照组,差异有统计学意义(P<0.05)。治疗组的抗生素使用天数短于对照组,差异有统计学意义(P<0.05)。两组在治疗过程中均未发生不良反应,治疗前后监测肝肾功能指标均未见异常。结论:中药贴敷治疗哺乳期急性乳腺炎的效果较为理想,可缩短患者抗生素使用时间,减轻炎症反应,且安全性高,值得在临床上推广。Objective:To observe the clinical effect of Chinese medicine application on lactation acute mastitis patients.Methods:This clinical trial prospective randomized controlled study approach,on December 1,2020 to July 31,2021 in urumqi friendly hospital hospitalization conforms to the mammary gland of lactation acute mammitis(acute simple inflammation)diagnostic criteria and TCM syndrome differentiation of gastric heat type indicates lag(using the stochastic indicator method),120 cases of patients were randomly divided into two groups,There were 60 cases in treatment group and 60 cases in control group.The treatment group was treated with cefuroxime sodium static drop+Chinese medicine paste+manual massage through milk,while the control group was treated with cefuroxime sodium static drop+manual massage through milk.The efficacy and clinical indexes of the two groups were compared.Results:On the second and third day after treatment,the range of breast inflammation in the treatment group was lower than that in the control group,and the levels of serum interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)and high-sensitivity C-reactive protein(hs-CRP)were lower than those in the control group,and the differences were statistically significant(P<0.05).The days of antibiotic use in the treatment group were shorter than those in the control group,and the difference was statistically significant(P<0.05).There were no adverse reactions in the two groups during the treatment,and no abnormal liver and kidney function indexes were observed before and after treatment.Conclusion:The effect of Chinese medicine application in the treatment of lactation acute mastitis is ideal,which can shorten the time of antibiotic use,reduce inflammation,and has high safety,which is worthy of clinical promotion.
