从湿热辨治皮肤病

摘　　要：当今湿热病理论与应用取得了长足发展,研究发现疑难皮肤病常与湿热之邪关系密切。湿病多因火热怫郁,水液停滞而发,因此湿者多自内生。湿病又分为外感湿邪、从化与合病、内生湿热。临床中善于查找病患湿热的特征,是治疗一些炎症性皮肤病的关键。治疗此类皮肤病需先清热利湿。湿热病特点为发病广泛、黏腻缠绵、传变多端、湿热成瘀,如带状疱疹、光敏性皮炎、掌跖脓疱病、远心性环状红斑、足癣等。治疗时均可从湿热辨证。治疗可在清热利湿基础上结合理气止痛、祛风止痒、凉血解毒等法。无论临床上表现为单一湿热病机或兼夹病机、复合病机,湿热都是诱发皮肤病的重要原因。从湿热辨证治疗皮肤病有利于找到治疗突破口,为有效治疗皮肤病提供依据,从而避免僵化的固定分型,执简驭繁,提高皮肤病的治疗效果。Nowadays,the theory and application of dampness and heat disease has made great progress.Studies have found that many refractory skin diseases are closely related to pathogenic factors of dampness and heat.Dampness diseases are mostly caused by fiery depression and water-dampness stagnation,so the dampness is mostly endogenous.Dampness diseases are further divided into exogenous dampness pathogens,metamorphosis,comprehensive diseases,and endogenous dampness and heat.It is the key to the treatment of some inflammatory skin diseases to be good at finding the dampness-heat characteristics of patients in clinical practice.To treat such skin diseases,it is necessary to clear away heat and remove dampness first.Dampness-heat disease is characterized by extensive onset,stickiness and lingering,variable transmission,and stasis of damp-heat.Representative diseases include herpes zoster,photosensitivity dermatitis,palmoplantar pustulosis,telecentric annular erythema,tinea pedis,etc.In the course of treatment,the disease can be differentiated from the aspect of dampness and heat.On the basis of clearing heat and dehumidifying,it can be combined with other treatments such as regulating Qi(气)and relieving pain,expelling wind and relieving itching,cooling blood and detoxifying.Whether it is clinically manifested as a single pathogenesis of damp-heat,a mixed pathogenesis or a compound pathogenesis,damp-heat is an important cause of skin diseases.Differentiating and treating skin diseases from the aspect of dampness-heat is conducive to finding breakthroughs in treatment,and providing a basis for the effective treatment of skin diseases,thereby avoiding rigid fixed classification,achieving the purposes of controlling many things by simple measures and methods and improving the effect.
