从后天脾胃论治吐酸病

摘　　要：吐酸病是指患者自觉有酸水自胃脘上泛于食管、咽喉或口腔的病症,临床上常伴有烧心、胸骨后烧灼痛,反酸或泛吐清水,嗳气反流,脘腹胀满,隐痛,食欲不振等症状,是消化系统的多发病和常见病。历代医家认为其寒热并存,病机以肝胃不和为主。文章着重论述“脾胃”在吐酸病中的地位,从后天脾胃论治吐酸病,指出本病多因宿食、湿痰、热郁、停饮等所致,突出“脾失健运、胃失和降”的病机特点,同时强调肝、肺协同致病的作用。并指出吐酸病的治疗应遵循“升清降浊、甘淡和胃”的原则,注意调理脾胃,辨证施治,采用不同的中医治疗方法以及适当的中医调护。Acid vomiting refers to the disease in which the patient is conscious to have acid water from the epigastric esophagus,throat or oral cavity.Clinically,it is often accompanied by symptoms such as heartburn,posterior sternal burning,acid regurgitation or vomiting water,belching,abdominal distension and fullness,dull pain,loss of appetite,etc.It is a frequent and common disease of the digestive system.Ancient medical experts believe that the cold and heat coexist in the disease,the pathogenesis is mainly liver and stomach disharmony.This paper focuses on the position of“spleen and stomach”in acid vomiting disease,and treating the disease from the acquired spleen and stomach,and points out that this disease is mostly caused by lodging food,wet phlegm,heat depression,water stagnation,etc.,which highlights the pathogenesis characteristics of“the spleen fails to transport and the stomach fails to harmonize”,and emphasizes the role of liver and lung in coordinating the pathogenesis.It is pointed out that the treatment of vomiting acid disease should follow the principle of“raising Qing(清)to reduce turbidity,harmonizing the stomach”,and paying attention to regulate the spleen and stomach,and different traditional Chinese medicine treatment methods and appropriate TCM nursing should be given.
