清肺健脾法对100例痰热壅肺型老年肺炎疗效及免疫功能的影响

摘　　要：目的:观察清肺健脾法对痰热壅肺型老年社区获得性肺炎的疗效及免疫功能的影响。方法:选择100例2018年12月-2020年1月在宁波市镇海区中医医院呼吸内科住院的痰热壅肺型老年社区获得性肺炎患者为研究对象,按照随机数字表法将患者分为观察组、对照组各50例。对照组予西医常规治疗,观察组在对照组基础上加服自拟清肺健脾汤,两组均治疗10日。观察两组治疗前后中医证候改善、T淋巴细胞亚群变化的情况,比较临床疗效。结果:治疗后两组中医症状总积分均较治疗前显著下降(P<0.01),且观察组治疗后的总积分低于对照组(P<0.05);治疗后对照组的CD3^(+)、CD4^(+)、CD8^(+)水平较治疗前无明显变化(P>0.05),但观察组的CD3^(+)、CD4^(+)水平较治疗前升高(P<0.05),CD8^(+)水平较治疗前下降(P<0.05);观察组的临床疗效优于对照组(P<0.05)。结论:清肺健脾法能显著缓解痰热壅肺型老年肺炎患者的临床症状,双向调节其T淋巴细胞亚群水平,改善细胞免疫功能,提高临床疗效,值得推广。Objective:To observe the effect of the Qingfei Jianpi therapy(清肺健脾法)on elder community-acquired pneumonia with phlegm-heat obstructing lung syndrome and its influence on immune function.Methods:A total of 100 cases from December 2018 to January 2020 were selected as the research objects.According to the random number table method,the patients were divided into the observation group and the control group,with 50 cases in each group.The control group was given conventional Western medicine,and the observation group was given the Qingfei Jianpi decoction(清肺健脾汤)more.Both of them were treated for 10 days.The improvement of TCM syndromes and the change of T-lymphocyte subsets were observed before and after treatment,and the clinical efficacy was compared.Results:After treatment,the scores of TCM symptoms in both groups were significantly lower than before treatment(P<0.01),and in the observation group were lower than the control group(P<0.05).After treatment,the levels of CD3^(+),CD4^(+)and CD8^(+)in the control group did not change significantly compared with before treatment(P>0.05),but the levels of CD3^(+)and CD4^(+)in the observation group were increased more compared with before treatment(P<0.05),and the level of CD8^(+)in the observation group was decreased compared with before treatment(P<0.05).The clinical efficacy in the observation group was better than the control group(P<0.05).Conclusion:The Qingfei Jianpi therapy can significantly relieve clinical symptoms,regulate the levels of T-lymphocyte subsets,improve cellular immune function and clinical efficacy,and is worth promoting.
