通痹舒筋丸对强直性脊柱炎患者外周血中IL-17、IL-23、TNF-α影响的临床研究

摘　　要：目的:验证通痹舒筋丸对强直性脊柱炎患者外周血中白细胞介素(Interleukin,IL)-17、IL-23、肿瘤坏死因子-α(Tumor Necrosis Factor-α,TNF-α)的用药效果。方法:将64例寒湿痹阻型强直性脊柱炎患者按照评定标准分为急性期组与慢性期组。两组患者均给予通痹舒筋丸口服,2个月为一个疗程;两组均治疗1个疗程。检测外周血中致炎因子IL-17、IL-23、TNF-α含量以及红细胞沉降率(Erythrocyte Sedimentation Rate,ESR)、C反应蛋白(C-reactive Protein,CRP)水平变化,观察治疗前后疼痛视觉模拟评分法(Visual Analogue Scale,VAS)评分、Bath强直性脊柱炎疾病活动指数(Bath Ankylosing Spondylitis Disease Active Index,BASDAI)评分的变化。结果:①急性期组和慢性期组患者治疗后的外周血中致炎因子IL-17、IL-23、TNF-α水平及BASDAI评分、VAS评分较治疗前明显下降,差异具有统计学意义(P<0.05);②急性期组患者治疗后ESR、CRP水平较治疗前明显降低,差异具有统计学意义(P<0.05),且两组患者治疗期间未出现明显的不良反应。结论:通痹舒筋丸可以明显降低寒湿痹阻型强直性脊柱炎患者外周血中IL-17、IL-23、TNF-α致炎因子水平,降低ESR、CRP炎症指标,减轻炎症反应,缓解疼痛,改善患者的临床症状。Objective:To explore the effect of Tongbi Shujin Wan(通痹舒筋丸)on the expression of IL-17,IL-23 and TNF-αin peripheral blood of patients with ankylosing spondylitis.Methods:Sixty-four patients with ankylosing spondylitis due to cold-damp impediment were divided into the acute phase group and the chronic phase group according to the evaluation criteria.Patients in the two groups were given Tongbi Shujin Wan orally,2 months as a course of treatment.Patients in the two groups received one course of treatment.The levels of proinflammatory cytokines IL-17,IL-23,TNF-α,ESR and CRP in peripheral blood were detected,and the VAS score and BASDAI score before and after treatment were observed.Results:①The levels of proinflammatory cytokines IL-17,IL-23,TNF-α,BASDAI and VAS scores in peripheral blood of patients in the acute phase group and the chronic phase group after treatment were significantly lower than those before treatment,and the differences were statistically significant(P<0.05).②After treatment,the levels of ESR and CRP of patients in the acute phase group were significantly lower than before,and the differences were statistically significant(P<0.05),and no significant adverse reactions were observed in the two groups during the treatment.Conclusion:Tongbi Shujin Wan can significantly reduce the levels of IL-17,IL-23,and TNF-α-induced inflammatory factors in peripheral blood of patients with ankylosing spondylitis due to cold-damp stagnation,it can reduce ESR and CRP inflammatory indicators,and decrease inflammatory reactions,thereby reducing pain and ameliorating clinical symptoms of patients.
