摘　　要：目的探究注射用胰蛋白酶联合关节镜手术对膝骨性关节炎患者疗效及IL-1β、MMP-3及TNF-α水平表达的影响。方法方便选择联勤保障部队第906医院骨科2019年2月—2020年12月收治的84例膝骨性关节炎患者开展研究,随机抽签法分为对照组、研究组,各42例。对照组进行关节镜手术+注射玻璃酸钠治疗,研究组进行关节镜手术+注射玻璃酸钠+注射用胰蛋白酶治疗,对比两组临床疗效(包括优良率、实验室指标水平、膝关节功能)。结果术后,研究组优良率95.24%较对照组80.95%高,差异有统计学意义(χ^(2)=4.086,P<0.05);研究组血清IL-1β、MMP-3及TNF-α水平较对照组低,差异有统计学意义(P<0.05);研究组膝关节评分系统(Hospital for Special Surgery,HSS)评分高于对照组,差异有统计学意义(P<0.05)。结论注射用胰蛋白酶联合关节镜手术方案疗效确切,可通过调节膝骨性关节炎患者血清IL-1β、MMP-3和TNF-α水平,改善膝关节功能,值得优选。Objective To explore the effect of trypsin injection combined with arthroscopic surgery on the curative effect and the expression of IL-1β,MMP-3 and TNF-in patients with knee osteoarthritis.Methods Conveniently selected from February 2019 to December 2020 in the Department of Orthopedics of the Joint Logistics Support Force 906 Hospital,84 patients admitted to the hospital with knee osteoarthritis were included in the study,and they were randomly divided into control group and study group,with 42 cases in each group.The control group received arthroscopic surgery+injection of sodium hyaluronate,and the study group received arthroscopic surgery+injection of sodium hyaluronate+trypsin for injection.Comparison of clinical efficacy between the two groups(including excellent rate,laboratory index level and knee function).Results After operation,the excellent and good rate of the study group was 95.24%,higher than control group 80.95%,the difference was statistically significant(χ^(2)=4.086,P<0.05);the serum levels of IL-1β,MMP-3 and TNF-in the study group were lower than those in the control group,the difference was statistically significant(P<0.05);the Hospital for Special Surgery(HSS)score of the study group was higher than that of the control group,the difference was statistically significant(P<0.05).Conclusion Trypsin for injection combined with arthroscopic surgery has a definite curative effect.It can improve the function of knee joint by regulating the levels of serum IL-1β,MMP-3 and TNF-αin patients with knee osteoarthritis,which is worthy of preference.
