摘　　要：目的 分析桥本甲状腺炎(HT)患者外周血淋巴细胞Fas/FasL水平与自身抗体水平的关系。方法 选取2018年2月至2020年9月于成都大学附属医院进行甲状腺检查的84例患者,根据病理情况分为HT组和单纯甲状腺肿(NTG)组,各有患者44例、40例;另外选取同期健康体检者45例作为对照组,检测三组的血清自身抗体水平、外周血淋巴细胞Fas/FasL水平并进行比较,采用Pearson相关性分析HT患者外周血淋巴细胞Fas/FasL以及自身抗体甲状腺球蛋白抗体(ATG)、甲状腺过氧化物酶抗体(ATPO)间的相关性。结果 三组血清自身抗体ATG、ATPO水平比较:HT组>NTG组、对照组,差异具有统计学意义(P<0.05),NTG组、对照组间ATG、ATPO比较差异无统计学意义(P>0.05)。三组FT3、FT4、TSH水平比较差异无统计学意义(P>0.05)。三组外周血淋巴细胞中Fas、FasL表达阳性率比较:HT组>NTG组>对照组,差异均有统计学意义(P<0.05)。Pearson相关性分析结果显示,HT患者外周血淋巴细胞Fas/FasL水平与ATG、ATPO表达成正相关(P<0.05)。结论 外周血淋巴细胞Fas/FasL表达水平与HT的发生存在一定关系,这可能与Fas/FasL可影响机体免疫生理过程有关。Objective To analyze the relationship between Fas/FasL in peripheral blood lymphocytes and autoantibody level in patients with Hashimoto’s thyroiditis(HT). Methods 84 patients who underwent thyroid examination in the Affiliated Hospital of Chengdu University from February 2018 to September 2020 were selected. According to pathological condition,the patients were divided into the HT group(44 cases)and the nontoxic goiter(NTG)group(40 cases). Meanwhile,45 healthy individuals were selected as the control group. Serum autoantibody levels and Fas/FasL in peripheral blood lymphocytes of the three groups were detected and compared. Pearson correlation analysis was performed to explore the correlation of Fas/FasL in peripheral blood lymphocytes,thyroglobulin antibody(ATG) and thyroid peroxidase antibody(ATPO)in patients with HT. Results The levels of serum ATG and ATPO in the HT group were significantly higher than those in the NTG group and the control group,with statistically significant differences(P<0.05),but there was no statistically significant difference between the NTG group and the control group(P>0.05).There was no statistically significant difference in FT3,FT4 or TSH level among the three groups(P>0.05).The positive expression rates of Fas and FasL in peripheral blood lymphocytes decreased in order from the HT group,the NTG group to the control group,with statistically significant differences(P<0.05). Pearson correlation analysis showed that Fas/Fas L level in peripheral blood lymphocytes of the HT group was positively correlated with the expression of ATG and ATPO(P<0.05). Conclusion There is a certain relationship between Fas/Fas L in peripheral blood lymphocytes and the occurrence of HT,which may be related to the effect of Fas/FasL on immune physiological process.
