复方大承气汤联合保留灌肠治疗粘连性肠梗阻临床观察  

Clinical observation on Compound Dachengqi Decoction combined with Retention Enema Therapy in the Treatment of Adhesive Intestinal Obstruction

摘　　要：目的探讨复方大承气汤联合保留灌肠法治疗粘连性肠梗阻患者的临床效果。方法将2017年3月—2019年5月就诊于江西省余干县中医院的80例粘连性肠梗阻患者作为研究对象,随机分为研究组与对照组,每组40例患者。对照组行复方大承气汤口服治疗,研究组在对照组治疗基础上加以中药保留灌肠治疗,比较分析2组患者治疗效果、症状改善情况及生活质量评分。结果研究组总有效率为95.00%(38/40),高于对照组的总有效率75.00%(30/40)(P<0.05);研究组肛门排气时间、肠蠕动恢复时间、腹胀及腹痛消除时间、住院时间均较对照组明显更短(P<0.05);入院时2组患者生活质量评分比较差异无统计学意义(P>0.05),出院时研究组生活质量评分较对照组明显更高(P<0.05)。结论复方大承气汤口服联合保留灌肠疗法有助于加快粘连性肠梗阻患者症状改善,提升其生活质量,临床疗效较为理想,可行性较高。Objective To investigate the clinical effect of compound Dachengqi decoction combined with retention enema in the treatment of adhesive intestinal obstruction.Methods Eighty patients with adhesive intestinal obstruction who were treated in Yugan Hospital of Traditional Chinese Medicine from March 2017 to May 2019 were randomly divided into study group and control group,with 40 cases in each group.The control group was treated with oral compound Dachengqi decoction,and the study group was treated with retention enema of Chinese medicine on the basis of treatment in the control group.The therapeutic effect,symptom improvement and quality of life were compared between the two groups.Results The total effective rate of the study group was 95.00%(38/40),which was higher than that of the control group 75.00%(30/40)(P<0.05).The time of anal exhaust,recovery time of intestinal peristalsis,elimination time of abdominal distension and abdominal pain,and hospital stay in the study group were significantly shorter than those in the control group(P<0.05).There was no significant difference in the quality of life score between the two groups at admission(P>0.05),but the quality of life score of the study group was significantly higher than that of the control group at discharge(P<0.05).Conclusion Oral administration of compound Dachengqi decoction combined with retention enema is helpful to accelerate the improvement of symptoms and improve the quality of life in patients with adhesive intestinal obstruction.The clinical effectis ideal and the feasibility is high.
