摘　　要：目的 探讨二维斑点追踪成像技术评价健康孕妇产后左心室心肌整体纵向应变的临床应用价值。方法 选取收治的80例健康孕妇按生产方式分为顺产组和剖宫产组,各组内再按年龄分为年轻组(A组,<35岁)和高龄组(B组,≥35岁),每组20例,同时选取健康未孕女性40例(未孕组),也按上述年龄段分为A组和B组,每组20例。对未孕女性进行超声心动图检查,对孕妇在产后1周内及6周进行超声心动图检查,分析左心室心肌整体纵向应变峰值(LVGLS),比较产后组和未孕组女性LVGLS的区别,分析孕妇在产后LVGLS的变化规律。结果 孕妇产后1周内各组与未孕组的LVGLS差异有统计学意义(P <0.05),孕妇产后6周各组与未孕组的LVGLS差异无统计学意义(P>0.05),顺产组与剖宫产组的LVGLS差异无统计学意义(P> 0.05),年轻组与高龄组孕妇产后的LVGLS差异无统计学意义(P> 0.05)。结论 健康孕妇产后早期左心室收缩功能有所减低,不同年龄、分娩方式对健康孕妇产后早期左心室收缩功能损害程度接近,二维斑点追踪成像具有无角度依赖性,可以及时发现孕妇产后左心室心肌功能异常,客观定量评价心肌局部收缩功能。Objective To explore the clinical value of two-dimensional speckle tracking imaging in evaluating the left ventricular global long-axis strain in postpartum women.Methods 80 healthy pregnant women admitted to our hospital were divided into spontaneous labor group and cesarean section group according to delivery method.Each group was divided into young group(group A,<35 years old)and old group(group B,≥35 years old)according to age,with 20 cases in each group.At the same time,40 healthy non-pregnant women were also divided into group A and group B according to the above age groups,with 20 cases in each group.The non-pregnant women were examined by echocardiography.The pregnant women were examined by echocardiography within 1 week and 6 weeks after delivery.The left ventricular global long-axis strain peak(LVGLS)was analyzed.The differences of LVGLS between the postpartum group and the non-pregnant group were compared,and the changes of LVGLS in the postpartum group were analyzed.Results There was significant difference in LVGLS between postpartum women and non-pregnant women within one week after delivery(P<0.05),there was no significant difference in LVGLS between postpartum women and non-pregnant women at 6 weeks postpartum(P>0.05),there was no significant difference in LVGLS between spontaneous labor group and cesarean section group(P>0.05),there was no significant difference in LVGLS between young group and old group(P>0.05).Conclusion Left ventricular systolic function of postpartum women has reduced in early stage different ages and delivery methods have similar degrees of damage to the early postpartum left ventricular systolic function of pregnant women.Two-dimensional speckle tracking imaging has no angle dependence.It can timely find the abnormal left ventricular myocardial function of postpartum women,objectively and quantitatively evaluate the local myocardial systolic function,and improve the postpartum health care level of pregnant women.
