摘　　要：目的探讨意外发现的活动性脊椎结核的^(18)F-FDG PET/CT表现并分析其对临床的应用价值。方法选取经病理证实的12例^(18)F-FDG PET/CT检查意外发现活动性脊椎结核的影像表现。结果经病理证实为脊椎结核的患者12例,共累及25个椎体,其中胸椎17个,腰椎6个,颈椎2个;同机CT图像可见19个椎体骨质破坏,1例可见椎旁脓肿,所有病例不能明确椎间盘受累;^(18)F-FDG代谢显示12例均呈^(18)F-FDG代谢增高,SUVmax为6.0~10.6,2例可见椎旁脓肿,从代谢情况显示所有病例均累及椎间盘。结论^(18)F-FDG PET/CT偶然发现脊椎高代谢病灶,需明确病灶是否累及椎间盘,如病灶累及椎间盘提示有活动性脊椎结核的可能性。Objective To investigate the ^(18)F-FDG PET/CT fingdings of active spinal tuberculosis and to analyze the clinical application.Methods The ^(18)F-FDG PET/CT imaging findings of 12 cases of active spinal tuberculosis were retrospectively analyzed.Results 12 cases of active spinal tuberculosis were confirmed by phthological findings,including 25 vertebras(17 thoracic vertebras,6 lumbar vertebras,and 2 cervical vertebras).In CT imagings,19 vertebras showed destruction of bone,1 case showed cold tuberculous abscess,and it could not show whether the intervertebral disc included.In ^(18)F-FDG PET/CT imagings,all cases showed high uptake of ^(18)F-FDG,and SUVmax was 6.0~10.6,2 cases showed cold tuberculous abscess,all cases showed the intervertebral disc involved.Conclusion ^(18)F-FDG PET/CT showed high uptake of ^(18)F-FDG accidentally,it should identify whether the intervertebral disc included,and if the intervertebral disc included,it was impossible of the diagnosis of active spinal tuberculosis.
