摘 要:目的 探讨骨性后踝撞击综合征的影像学特征。方法 选取我院疑似骨性后踝撞击综合征患者97例,均行关节镜确诊后再行X线摄影和MRI检查。结果 疑似骨性后踝撞击综合征患者97例,经关节镜诊断确诊骨性后踝撞击综合征患者84例。经X线摄影诊断骨性后踝撞击综合征准确率为59.79%。经MRI诊断骨性后踝撞击综合征准确率为74.23%。MRI诊断骨性后踝撞击综合征准确率高于X线摄影,差异有统计学意义(P <0.05)。MRI诊断骨性后踝撞击综合征灵敏度高于X线摄影,差异有统计学意义(P <0.05);MRI与X线摄影诊断骨性后踝撞击综合征特异度比较差异无统计学意义(P> 0.05);MRI诊断Kappa值高于X线摄影。结论 MRI诊断骨性后踝撞击综合征准确率优于X线摄影,且诊断灵敏度高。Objective To investigate the imaging features of osteogenic posterior malleolar impingement syndrome.Methods 97 patients with suspected bony posterior malleolar impingement syndrome in our hospital from March 2019 to March 2022 were selected as the study subjects.All of them were diagnosed by arthroscopy,X-ray and MRI.The technical operators were operated by two imaging diagnostic physicians in the imaging department(with more than 8 years of working experience),who can skillfully operate the machine and identify suspicious lesions of the posterior ankle,and jointly complete the MRI standard examination sequence.Results There were 97 patients with suspected bone posterior malleolar impingement syndrome,and 84 patients with bone posterior malleolar impingement syndrome were diagnosed by arthroscopy.The accuracy of X-ray diagnosis of bony posterior malleolar impingement syndrome was 59.79%.The accuracy of MRI in the diagnosis of bony posterior malleolar impingement syndrome was 74.23%.The accuracy of MRI in the diagnosis of bony posterior malleolar impingement syndrome was higher than that of X-ray(P<0.05).The sensitivity of MRI in the diagnosis of bony posterior malleolar impingement syndrome was higher than X-ray(P<0.05);There was no significant difference in the specificity of MRI and X-ray in the diagnosis of bony posterior malleolar impingement syndrome(P>0.05);the Kappa value of MRI was higher than that of X-ray.Conclusion MRI is superior to X-ray in the diagnosis of bony posterior malleolar impingement syndrome,with high sensitivity and good consistency.
