摘 要:目的 了解肠杆菌科细菌感染的临床特征及耐药特点,为感染性疾病的经验治疗提供实验依据。方法 回顾性分析2019年至2021年住院患者细菌培养结果为肠道杆菌的临床资料。结果 共分离出肠道杆菌579株,检出率排前三位的依次是肺炎克雷伯菌(38.34%)、大肠埃希菌(30.92%)和阴沟肠杆菌(9.84%)。主要感染部位是下呼吸道(56.13%)和泌尿道(15.89%)。产超广谱β-内酰胺酶(ESBLs)的大肠埃希菌及肺炎克雷伯菌检出率分别为57.54%和28.38%,ESBLs阳性菌株耐药性严重,未检出亚胺培南耐药和美洛培南耐药菌株,较敏感的药物还有丁胺卡那、替卡西林/他唑巴坦。结论 肠道杆菌主要以肺炎克雷伯菌和大肠埃希菌为主,最常见的感染部位是下呼吸道,产酶菌株耐药严重,临床应根据药敏结果制定有效的治疗方案,提高治愈率。Objective To understand the clinical characteristics and drug resistance of Enterobacteriaceae infection,and to provide experimental basis for the empirical treatment of infectious diseases.Methods The clinical data of inpatients whose bacterial culture results were Enterobacteriaceae from 2019 to 2021 were retrospectively analyzed.Results A total of 579 strains of Enterobacteriaceae were isolated.The first three strains were Klebsiella pneumoniae(38.34%),Escherichia coli(30.92%) and Enterobacter cloacae(9.84%).The main infection sites were lower respiratory tract(56.13%) and urinary tract(15.89%).The detection rates of Escherichia coli and Klebsiella pneumoniae producing extended spectrum β-Lactamase(ESBLs) were 57.54% and 28.38% respectively.ESBLs positive strains had serious drug resistance,and imipenem resistant and meropenem resistant strains were not detected.Amikacin and ticarcillin/tazobactam were also more sensitive drugs.Conclusion Klebsiella pneumoniae and Escherichia coli are the main Enterobacteriaceae.The most common infection site is the lower respiratory tract.The enzyme producing strains have serious drug resistance.Effective treatment schemes should be formulated according to the drug sensitivity results to improve the cure rate.
