支气管哮喘患儿外周血miR-123-P13K/AKT信号通路的表达及临床意义

摘  要:目的:探讨支气管哮喘患儿外周血miR-123-磷脂酰肌醇3激酶蛋白激酶B(P13KAKT)信号通路的表达及临床意义。方法:选取2019年1月-2020年12月深圳市儿童医院收治的支气管哮喘患儿50例(研究组),同期选择体检健康儿童50例(对照组)。采用RT-qPCR法检测患儿外周血miR-123-P13K、AKTmRNA表达。同期采用ELISA法检测两组患儿血清白细胞介素4(IL-4)和干扰素γ(IFN-γ)的表达情况。采用Pearson相关系数进行血清miR-123表达的相关性分析。采用多元线性回归方程及logistics回归模型进行回归分析。结果:研究组miR-123、P13K、AKTmRNA、IL-4均显著高于对照组,而IFN-γ、最大呼气流量(PEF)、第一秒呼出的气体总量(FEV1)和肺活量(VC)显著低于对照组,差异有统计学意义(t=22.41、2.32、28.8、19.45、18.7、50.89、47.53、6.85,P<0.05)。miR-123 mRNA的表达与P13K、AKT、IL-4呈显著正相关,差异有统计学意义(r=0.69、0.67、0.61,P<0.05),而与IFN-γ、PEF、FEV1和VC显著负相关,差异有统计学意义(r=-0.60、-0.54、-0.57,P<0.05)。多元线性回归结果发现P13K、AKT、IL-4、IFN-γ、PEF、FEV1和VC为影响其表达的独立危险因素。多因素logistics回归分析结果显示,miR-123、IFN-γ、IL-4、AKT、P13K和肺功能为独立危险因素。结论:支气管哮喘患儿外周血miR-123-P13K/AKT信号通路的表达明显上调,临床可通过检测去表达水平评估预测患儿病情发展。Objective:To investigate the expression and clinical significance of miR-123-P13KAKT signaling pathway in peripheral blood of children with bronchial asthma.Methods:A total of 50 children with bronchial asthma admitted to the hospital from January 2019 to December 2020 were selected(study group),and 50 healthy children with physical examination(control group)were selected during the same period.The expression of miR-123 P13K and AKT mRNA in peripheral blood of children was detected by RT-qPCR.During the same period,ELISA was used to detect the expressions of serum IL-4 and IFN-γin the two groups.The correlation analysis of serum miR-123 expression was performed by Pearson correlation coefficient,and the regression analysis was performed by multiple linear regression equation and logistic regression model.Results:The levels of miR-123,PI3K,AKT mRNA and IL-4 in the study group were significantly higher than those in the control group,while IFN-γ,PEF,FEV1 and VC were significantly lower than those in the control group,and the differences were statistically significant(t=22.41,2.32,28.8,19.45,18.7,50.89,47.53,6.85,P<0.05).The expression of miR-123 mRNA was significantly positively correlated with P13K,AKT and IL-4,and the difference was statistically significant(r=0.69,0.67,0.61,P<0.05),while it was negatively correlated with IFN-γ,PEF,FEV1 and VC,and the difference was statistically significant(r=-0.60,-0.54,-0.57,P<0.05,P<0.05).Multiple linear regression results showed that P13K,AKT,IL-4,IFN-γ,PEF,FEV1 and VC were independent risk factors affecting its expression.Multivariate logistic regression analysis showed that miR-123,IFN-γ,IL-4,AKT,P13K and lung function were independent risk factors.Conclusion:The expression of miR-123-P13KAKT signaling pathway in peripheral blood of children with bronchial asthma is significantly up-regulated,and the clinical evaluation of the de-expression level can predict the development of children’s disease.
