摘 要:目的探讨经幽门喂养联合暖箱对极低出生体质量早产儿(VLBWI)喂养耐受性及呼吸暂停的影响。方法选取2020年4月至2021年10月医院出生的86例VLBWI为研究对象,按随机数字表法将患儿分为对照组与观察组,每组43例。将两组均放入暖箱中,对照组采用经胃管喂养,观察组采用经幽门喂养,比较两组喂养不耐受发生率、生长发育指标、呼吸暂停发生情况、住院时间及胃肠功能。结果观察组喂养不耐受发生率低于对照组,差异有统计学意义(P<0.05);观察组达全肠道喂养时间短于对照组,出生后14 d体质量、头围、身长增长水平均高于对照组,差异有统计学意义(P<0.05);观察组呼吸暂停发生次数少于对照组,住院时间短于对照组,差异有统计学意义(P<0.05);观察组听诊器听胃肠蠕动、肠鸣音均多于对照组,差异有统计学意义(P<0.05)。结论经幽门喂养联合暖箱可显著提升VLBWI的喂养耐受性,减少呼吸暂停发生,并有利于促进患儿机体发育。Objective To explore the effect of pyloric feeding combined with incubator on feeding tolerance and apnea of very low birth weight premature infants(VLBWI).Methods A total of 86 patients with VLBWI born in hospital from April 2020 to October 2021 were selected as the research objects,and they were randomly divided into control group and observation group,with 43 cases in each group.Both groups were placed in a warm box.The control group was fed via gastric tube,while the observation group was fed via pylorus.The incidence of feeding intolerance,growth and development indicators,apnea,hospitalization time and gastrointestinal function were compared between the two groups.Results The incidence of feeding intolerance in observation group was lower than that in control group,and the difference was statistically significant(P<0.05).The whole intestinal feeding time of the observation group was shorter than that of the control group,and the growth level of body weight,head circumference and body length on 14th day after birth was higher than that of the control group,with statistical significance(P<0.05).The frequency of apnea in the observation group was less than that in the control group,and the hospitalization time was shorter than that in the control group(P<0.05).The number of gastrointestinal peristalsis and bowel sounds heard by stethoscope in observation group was higher than that in control group,and the difference was statistically significant(P<0.05).Conclusion Pyloric feeding combined with incubator can significantly improve the feeding tolerance of VLBWI,reduce the occurrence of apnea,and promote the development of children.
