达格列净联合胰岛素对2型糖尿病患者肾功能及颈动脉内膜中层厚度、脂蛋白相关磷脂酶A2的影响

摘　　要：目的探讨达格列净联合胰岛素对2型糖尿病患者肾功能及颈动脉内膜中层厚度(CIMT)、脂蛋白相关磷脂酶A2(Lp-PLA2)的影响。方法选择82例2型糖尿病患者,随机数表法分为研究组与对照组,各41例。对照组采用原有降糖方案或联用胰岛素治疗,研究组在对照组基础上予达格列净治疗,2组治疗时间均为6个月。比较2组空腹血糖、糖化血红蛋白,血清胱抑素C(CysC)、β_(2)微球蛋白(β_(2)-MG),CIMT、Lp-PLA2水平及不良反应发生情况。结果治疗后2组FBG、HbA1c水平均降低(P<0.05),且研究组FBG、HbA1c水平均低于对照组(P<0.05);治疗后2组血清CysC、β_(2)-MG水平均降低(P<0.05),且研究组血清CysC、β_(2)-MG水平均低于对照组(P<0.05);治疗后2组CIMT、Lp-PLA2均降低(P<0.05),且研究组CIMT、Lp-PLA2均低于对照组(P<0.05);2组不良反应发生率比较,差异无统计学意义(P>0.05)。结论达格列净联合胰岛素治疗2型糖尿病患者能够改善血糖水平,保护肾功能,降低CIMT及Lp-PLA2水平,安全性较高。Objective To explore the effects of dapagliflozin combined with insulin on the renal function,carotid intima-media thickness (CIMT) and lipoprotein-related phospholipase A2 (Lp-PLA2) in patients with type 2 diabetes.Methods A total of 82 patients with type 2 diabetes who were admitted to the hospital from January 2018 to January 2021 were selected and divided into a study group and a control group according to the random number table method,with 41 cases in each group.The control group was treated with the original hypoglycemic regimen or combined with insulin,while the study group was treated with dapagli?ozin on the basis of the control group,both for 6 months.The levels of fasting blood glucose,glycosylated hemoglobin,serum cystatin C (CysC),βmicroglobulin (β-MG),CIMT,Lp-PLA2 and adverse reactions were compared between the two groups of patients.Results After treatment,the levels of FBG and HbA1c were decreased in both groups (P<0.05),and the study group was lower than the control group (P<0.05).After treatment,the levels of serum CysC,β-MG were decreased in both groups (P<0.05),and the study group was lower than the control group (P<0.05).After treatment,the levels of CIMT and Lp-PLA2 were decreased in both groups (P<0.05),and the study group was lower than the control group (P<0.05).There were no significant differences in the incidence of adverse reactions between the two groups (P>0.05).Conclusion Daggligin combined with insulin can improve the level of blood glucose,protect the renal function,reduce the levels of CIMT and Lp-PLA2 in patients with type 2 diabetes,which is safe and reliable.
