代谢综合征患者腰围及热应激蛋白70与血清肿瘤坏死因子-α水平的关联研究

摘　　要：目的探讨代谢综合征(MS)患者腰围、热应激蛋白70(HSP-70)与血清肿瘤坏死因子(TNF-α)水平的关联和相互作用。方法于2015年3月至2016年3月从宁夏医科大学总医院及吴忠市人民医院,按照纳入与排除标准,选取无血缘关系的376例MS患者,通过问卷调查研究对象的姓名、性别、年龄、生活史、家族史及MS相关疾病史等一般情况,测量身高、体质量等指标并检测血液白细胞计数、血糖等实验室指标。分析MS患者在不同TNF-α水平下对各指标的影响;以腰围(WC)和HSP-70为自变量,以血清TNF-α为因变量,构建多元线性回归方程;采用偏相关分析WC和HSP-70相关性。结果不同性别MS患者间年龄、体质量指数(BMI)、WC、臀围(HC)、腰臀比(WHR)、三酯甘油(TG)、总胆固醇(TC)、高密度脂蛋白胆固醇(HDL-C)、低密度脂蛋白胆固醇(LDL-C)、收缩压(SBP)、天门冬氨酸氨基转移酶(AST)、丙氨酸氨基转移酶(ALT)、空腹血糖(FPG)、白细胞(WBC)、HSP-70和尿酸(UA)在两组间的分布差异均无统计学意义(P均>0.05),女性的DBP稍低于男性(P=0.002)。在不同的血清TNF-α水平间,MS患者的WC(P=0.046)、WHR(P=0.043)、HSP-70(P<0.001)和UA(P=0.045)差别均有统计学意义。TNF-α与WC、HSP-70之间存在线性关系,调整R2=0.101,线性回归方程为Y_(TNF-α)=13.131-0.053X_(HSP-70)+0.045X(WC)(P<0.05)。WC和HSP-70在控制血清TNF-α后无相关性(r=0.094,P=0.071)。结论MS患者的血清TNF-α水平随着WC水平增加和HSP-70表达减少而升高,控制TNF-α后WC和HSP-70无相关性。Objective To explore the relationship and interaction between serum TNF-αand waist circumference,heat stress protein 70 in patients with metabolic syndrome.Methods From March 2015 to March 2016,according to the inclusion and exclusion criteria,376 patients with MS without blood relationship were selected from the General Hospital of Ningxia Medical University and the People's Hospital of Wuzhong City,and their names,gender,age,life history,family history,MS-related disease history and other general conditions were investigated by questionnaires,measure their height,weight and other physical indicators,and their blood samples were collected in the morning for leukocyte count,blood sugar and other laboratory indicators.The effects of TNF-αlevels on the indexes of patients were analyzed,the multiple linear regression equations were constructed with WC and HSP-70 as independent variables and serum TNF-αas dependent variables,and partial correlation analysis of WC and HSP-70 was used.Results There were no significant differences in age,BMI,WC,HC,WHR,TG,TC,HDL-C,LDL-C,SBP,AST,ALT,FPG,WBC,HSP-70 and UA between the two groups(P all>0.05),and DBP in women was slightly lower than that in men(P=0.002).There were statistically significant differences in WC(P=0.046),WHR(P=0.043),HSP-70(P<0.001)and UA(P=0.045)among different serum levels of TNF-αin MS patients.There was a linear relationship between TNF-αand WC,HSP-70.Adjusted R2=0.101,the linear regression equation was Y_(TNF-α)=13.131-0.053X_(HSP-70)+0.045X_(WC)(P<0.05).No correlation between WC and HSP-70 without TNF-α(r=0.094,P=0.071).Conclusion The serum level of TNF-αincreased with the rise of WC and the decrease of HSP-70 expression in MS patients,but there was no correlation between WC and HSP-70 without TNF-α.
