摘　　要：目的:探讨Stanford A型主动脉夹层术后并发症的发生率,为术中设计以及术后干预的准备提供科学依据。方法:检索PubMed、CNKI、WANFANG DATA、CQVIP等数据库所收录的文献,应用R 4.0.2统计学软件对纳入的文献进行Meta分析。结果:纳入文献11篇,共1181例,发生并发症423例。Meta分析结果显示,急性主动脉夹层术后并发症总发生率为34.70%(95%CI:27.33%~44.06%),一过性脑功能紊乱发生率为10.58%(95%CI:7.84%~14.89%),脊髓损伤发生率为2.13%(95%CI:1.07%~3.47%),脑梗死发生率为6.23%(95%CI:3.72%~9.21%),肾衰竭发生率为7.42%(95%CI:5.28%~9.88%)。结论:主动脉夹层术后并发症发生率高、种类多,在临床路径设计上尤其应注意避开神经系统综合征和肾衰竭的相关危险因素。Objective:To investigate the incidence of postoperative complications in Stanford type A aortic dissection,and to provide scientific basis for intraoperative design and preparation of postoperative intervention.Methods:The literatures included in PubMed,CNKI,WANFANG DATA,CQVIP and other databases were searched,and R version 4.0.2 was used for Meta-analysis.Results:A total of 11 literatures were included,423 cases had complications,and 1181 cases were included.Meta-analysis showed that the overall incidence of complications after acute aortic dissection was 34.70%(95%CI:27.33%to 44.06%),transient brain dysfunction was 10.58%(95%CI:7.84%to 14.89%),spinal cord injury was 2.13%(95%CI:1.07%to 3.47%),cerebral infarction was 6.23%(95%CI:3.72%to 9.21%),and renal failure was 7.42%(95%CI:5.28%to 9.88%).Conclusion:The incidence of postoperative complications in aortic dissection is high and there are many kinds.In clinical pathway design,special attention should be paid to avoid the risk factors related to neurological syndrome and renal failure.
