带线锚钉与可吸收缝线重建喙锁韧带联合锁骨钩钢板治疗陈旧性肩锁关节脱位的临床疗效

摘　　要：目的探讨带线锚钉与可吸收缝线重建喙锁韧带联合锁骨钩钢板治疗陈旧性肩锁关节脱位的临床疗效。方法采用回顾性研究,选择2020年10月~2022年2月武威市人民医院收治的陈旧性肩锁关节脱位患者60例,按照内固定方式不同分为观察组(喙锁韧带重建联合锁骨钩钢板)和对照组(单纯肩锁关节清理固定术),各30例。对比两组手术疗效、手术时间及术后恢复情况、术后3个月肩关节功能Constant-Murley评分、视觉模拟评分(VAS评分)、喙锁间距(CCD)和肩锁间隙(ACD)。结果观察组优良率96.67%(29/30)显著高于对照组73.33%(22/30),差异有统计学意义(P<0.05)。观察组手术时间长于对照组,差异有统计学意义(P<0.05)。观察组骨折愈合时间、关节活动恢复时间和并发症发生率均显著少于对照组,差异有统计学意义(P<0.05)。观察组Constant-Murley评分显著大于对照组,差异有统计学意义(P<0.05)。观察组VAS评分显著低于对照组,差异有统计学意义(P<0.05)。两组CCD和ACD比较,差异无统计学意义(P>0.05)。结论带线锚钉与可吸收缝线重建喙锁韧带联合锁骨钩钢板治疗陈旧性肩锁关节脱位效果显著,可有效促进骨折愈合、尽早恢复肩关节功能,减轻疼痛,且并发症少,值得临床应用。Objective To investigate the clinical effect of reconstruction of coracoclavicular ligament with suture anchor and absorbable suture combined with clavicular hook plate in the treatment of old acromioclavicular joint dislocation.Methods A retrospective study was adopted,a total of 60 patients with old acromioclavicular joint dislocation who were admitted to Wuwei People’s Hospital from October 2020 to February 2022 were selected and divided into observation group(coracoclavicular ligament reconstruction combined with clavicle hook plate)and control group(Simple acromioclavicular joint debridement and fixation)30 cases in each group.The operative efficacy,operative time,postoperative recovery,Constant-Murley score of shoulder joint function,visual analogue scale(VAS)score,coracoclavicular distance(CCD)and acromioclavicular space(ACD)were compared between the two groups.Results The excellent and good rate of the observation group was 96.67%(29/30),which was signifi cantly higher than that of the control group,73.33%(22/30),the difference was statistically significant(P<0.05).The operation time of the observation group was longer than that of the control group,the diff erence was statistically signifi cant(P<0.05).The Constant-Murley score of the observation group was signifi cantly higher than that of the control group,the diff erence was statistically signifi cant(P<0.05).The VAS score was signifi cantly lower than that of the control group,the diff erence was statistically signifi cant(P<0.05).There was no diff erence in CCD and ACD between the two groups(P>0.05).Conclusion The reconstruction of coracoclavicular ligament with suture anchor and absorbable suture combined with clavicular hook plate is eff ective in the treatment of old acromioclavicular joint dislocation.It can eff ectively promote fracture healing,restore shoulder function as soon as possible,reduce pain,and have fewer complications,which is worthy of clinical application.
