摘  要:寻常型白癜风是日常生活中常见的一种皮肤黏膜发生色素脱失的疾病。其发病机制有多种解释,但临床和实验结果大多以自身免疫理论为主。由T淋巴细胞介导的自身免疫应答在其中发挥重要作用。辅助型T细胞1(Th1)、Th2、Th17和调节性T细胞(Treg)是CD4^(+)T细胞的重要亚群,对引起寻常型白癜风发挥不可或缺的作用,其中Th1和Th2细胞因子的产生失衡与之尤为相关。主要源于Th1的IL-2和IFN-γ对Th1介导的细胞免疫发挥关键作用。IL-2对Th17及Treg的发展及Th7/Treg细胞平衡的恢复均发挥关键作用。体液免疫主要由Th2介导,IL-4可诱导Th0分化为Th2,增强体液免疫应答能力。本文概述了IL-2、IL-4及IFN-γ在寻常型白癜风免疫机制中的作用。Vitiligo vulgaris is a common acquired skin disease characterized by the loss of pigment in skin mucosa. There are several theories to disclose pathogenesis of vitiligo vulgaris,but the clinical and experimental results tend to be autoimmunity theory,T lymphocyte mediated autoimmunity plays an important role in pathogenesis of vitiligo vulgaris. helper T cells 1(Th1),Th2,Th17and regulatory T cells(Treg)are important subsets of CD4^(+)T cells that play an important role in pathogenesis of vitiligo vulgaris,imbalance of Th1 and Th2 cytokines is highly correlated with occurrence and development of many autoimmune diseases. Th1 mainly mediate cellular immunity,with Th1-derived IL-2 and IFN-γ playing a key role in it. IL-2 is instrumental to development of Th17 and Treg and the restoration of Th7/Treg cell balance. Humoral immunity is mainly mediated by Th2 cell,and IL-4 can induce Th0 to differentiate into Th2,enhancing humoral immune response. Role of IL-2,IL-4 and IFN-γ in pathogenesis of vitiligo vulgaris was reviewed in this paper.
