Ⅰ期与Ⅱ期输尿管软镜钬激光碎石治疗肾结石的效果对比分析

摘　　要：目的:探讨Ⅰ期输尿管软镜钬激光碎石(FURS)与Ⅱ期FURS治疗肾结石的效果。方法:回顾性分析上海中医药大学附属普陀医院2021年1月—12月收治的65例肾结石长径在1~2cm的行FURS患者,其中30例行Ⅰ期FURS患者纳入A组,35例行Ⅱ期FURS纳入B组。统计并比较两组患者年龄、结石大小、手术时间、术后两周清石率、术前术后肾功能(尿素氮、肌酐)、炎症指标[血尿常规白细胞、C反应蛋白(CRP)、降钙素原(PCT)]。结果:B组手术时间短于A组,差异有统计学意义(P<0.001);B组肾功能影响小于A组,差异有统计学意义(P<0.001)。但两组炎症指标和清石率比较,差异无统计学意义(P>0.05);两组均没有严重并发症。结论:治疗长径2cm以内的小负荷肾结石,相较于Ⅱ期FURS,Ⅰ期FURS短期内肾功能影响较大,手术时间较长,但治疗周期更短,更节省材料和费用。Objective To investigate the effects of FURS treatment of stage I and stage II.Methods A total of 65 cases admitted to Putuo Hospital affiliated to Shanghai University of Traditional Chinese Medicine from January to December 2021 of FURS were retrospectively reviewed.Among them,30 cases were stage I FURS admitted to Group A while 35 cases were stage II FURS admitted to Group B.The patient's age,stone size,operation time,stone clearance rate(two weeks after operation by CT),renal function before and after operation(blood urea nitrogen,creatinine),inflammatory indicators(white blood cells of blood and urine routine,C-reactive protein CRP,procalcitonin PCT)were counted.Results In the treatment of upper urinary tract stones with stage I FURS and stage II FURS,the operation time in group B was shorter than that in group A(P<0.001),the effect of renal function in group B was less than that in group A(P<0.001).But in terms of inflammatory indicators and stone clearance rate,the difference between the two methods was not statistically significant.Of course,there was no serious complication in both groups.Conclusion Compared with stage II FURS,stage I FURS had a greater impact on renal function and longer operation time in the treatment of small-load stones within 2 cm in length,however the stone clearance rate,inflammatory indicators and stage II FURS were not statistically significant.Despite the defect,stage I FURS had the shorter time during the treatment,it also could save materials and costs.
