摘　　要：目的探讨胆囊部分切除在腹腔镜复杂胆囊手术中的临床应用价值。方法收集2018年6月至2022年4月在河池市人民医院行腹腔镜手术治疗的80例复杂胆囊患者作为研究对象,根据随机数表法分为研究组和对照组,每组各40例。研究组采用腹腔镜胆囊部分切除术,对照组采用腹腔镜胆囊切除术。比较两组患者围手术期指标、中转开腹手术率、手术并发症及临床疗效。结果研究组术中出血量、术后腹腔引流量少于对照组,手术时间、引流管留置时间、术后下床活动时间、术后住院时间短于对照组,研究组中转开腹手术率和手术并发症总发生率低于对照组,研究组手术治疗总有效率高于对照组(P<0.05)。结论腹腔镜胆囊部分切除能减少复杂胆囊患者的术中出血量,降低中转开腹手术率和手术并发症的发生率,提高临床疗效,是一种安全、可行的微创手术方式,值得临床推广应用。Objective To investigate the clinical application value of partial cholecystectomy in laparoscopic complicated gallbladder operation.Methods A total of 80 patients with complicated gallbladder who underwent laparoscopic surgery admitted to the People’s Hospital of Hechi from June 2018 to April 2022 were collected as the research subjects,and they were divided into the study group(n=40)and the control group(n=40)according to the random number table method.The study group was treated with laparoscopic partial cholecystectomy,while the control group was treated with laparoscopic cholecystectomy.The perioperative indices,conversion to open cholecystectomy,surgical complications and clinical efficacy were compared between the patients in the two groups.Results The intraoperative hemorrhage volume and postoperative peritoneal drainage volume in the study group were less than those in the control group,and the operation time,the retention time of drainage tube,the time of getting out of bed after operation and hospitalization time after operation were shorter than those in the control group.The conversion rate to open cholecystectomy and the total incidence of surgical complications in the study group were lower than those in the control group,and the overall response rate of surgical treatment in the study group was higher than that in the control group(P<0.05).Conclusion Laparoscopic partial cholecystectomy can reduce the intraoperative hemorrhage volume in patients with complicated gallbladder,reduce the conversion rate to open cholecystectomy and the incidence of surgical complications,and improve the clinical efficacy.Therefore,it is a safe and feasible minimally invasive surgical method,which is worthy of clinical promotion and application.
