微创踝关节融合术治疗创伤性关节炎的效果及对患者远期足踝功能和生活质量的影响

摘　　要：目的探究微创踝关节融合术用于治疗足踝创伤性关节炎的临床效果。方法选择2015年2月至2019年2月解放军第900医院仓山院区诊治的90例足踝创伤性关节炎患者,使用随机数表法将患者分为对照组(45例)及研究组(45例)。对照组使用开放式踝关节融合术治疗,研究组使用微创踝关节融合术治疗。比较两组患者骨性愈合情况、骨性愈合时间、治疗前后踝-后足功能评分情况和疼痛评分及生活质量测定量表评分情况。结果治疗后,研究组术后6个月骨性愈合率高于对照组(P<0.05),骨性愈合时间短于对照组(P<0.05);研究组踝-后足功能评分比对照组更高(P<0.05),视觉模拟疼痛评分比对照组更低(P<0.05);研究组的生活质量评分比对照组更高(P<0.05)。结论将微创踝关节融合术应用于足踝创伤性关节炎患者治疗中,能提高骨性愈合率,改善足踝关节功能,缓解疼痛感,提升患者生活质量水平。Objective To explore the clinical efficacy of minimally invasive ankle arthrodesis in the treatment of foot and ankle traumatic arthritis.Methods A total of 90 patients with foot and ankle traumatic arthritis treated in the CangShan Dis trict of the 900th H ospital of from February 2015 to February 2019 were selected.The patients were divided into the control group(45 cases)and the study group(45 cases)by random number table method.The control group was treated with open ankle arthrodesis,while the study group was treated with minimally invasive ankle arthrodesis.The osseous union,osseous union time,ankle-hindfoot function score,pain score and quality of life scale score before and after treatment were compared between the two groups.Results After treatment,the bone healing rate of the study group was higher than that of the control group at 6 months after operation(P<0.05),and the bone healing time was shorter than that of the control group(P<0.05).The ankle-hindfoot function score of the study group was higher than that of the control group(P<0.05),and the visual analogue pain score was lower than that of the control group(P<0.05).The quality of life score of the study group was higher than that of the control group(P<0.05).Conclusion The application of minimally invasive ankle fusion in the treatment of patients with traumatic arthritis can improve the rate of bone healing,improve the function of the ankle joint,relieve pain,and improve the quality of life of patients.
