摘 要:目的 探究冲击波联合持续被动活动(CPM)下肢关节功能康复器治疗下肢骨折后膝关节功能障碍的临床效果。方法 将90例下肢骨折后膝关节功能障碍患者按照随机数字表法均分为对照组和观察组,对照组予以康复锻炼联合CPM治疗,观察组在对照组基础上加用放射状冲击波(RSWT)治疗,连续治疗3周,比较两组治疗总有效率,治疗前后膝关节功能、负性情绪以及生活质量,并统计不良反应发生情况。结果 治疗3周后,观察组总有效率(97.78%)高于对照组(77.78%)(P <0.05);观察组视觉模拟评分量表(VAS)评分低于对照组,膝关节活动度、Lysholm膝关节评分均高于对照组(P <0.05);观察组焦虑自评量表(SAS)、抑郁自评量表(SDS)均低于对照组(P <0.05);观察组生理功能评分、生理职能评分、躯体疼痛评分、总体健康评分、活力评分、社会功能评分、情感职能评分及心理健康评分均高于对照组(P <0.05)。结论 冲击波联合CPM治疗下肢骨折后膝关节功能障碍患者的疗效更高,可显著提升膝功能,缓解负性情绪,提高生活质量。Objective To explore the clinical effect of shock wave combined with continuous passive motion(CPM)lower limb joint function recovery device on knee dysfunction after lower limb fracture.Methods Ninety patients with knee dysfunction after lower limb fracture were all divided into control and observation groups according to the method of randomized number table.The control group received rehabilitation exercise combined with CPM treatment,and the observation group was treated with radioactive shock wave(RSWT)based on the control group for 3 consecutive weeks.The rates of treatment total response,knee joint function,negative mood and quality of life before and after treatment were compared between the two groups.The occurrence of adverse reactions was also counted.Results After 3 weeks of treatment,the total response rate(97.78%)was higher than the control group(77.78%,P<0.05).The visual simulation rating scale(VAS)score of the observation group was lower than that of the control group.Knee mobility and Lysholm knee score were higher than the control group(P<0.05).The observation anxiety self-rating scale(SAS)and depression self-rating scale(SDS)were all lower than those of the control group(P<0.05).The physical function score,physical function score,physical pain score,overall health score,vitality score,social function score,emotional function score and mental health scores were all higher than those of the control group(P<0.05).Conclusion Shock wave combined with CPM has a higher efficacy in treating patients with knee dysfunction after lower limb fracture,which can significantly improve the knee function,relieve negative mood and improve the quality of life.
