慢性收缩性心力衰竭患者超声心动图指标与TyG指数、RDW和Cys-C的关系

摘　　要：目的探讨慢性收缩性心力衰竭(CSHF)患者超声心动图指标左室舒张末内径(LVED)、左室射血分数(LVEF)、左室短轴缩短率(LVFS)与空腹甘油三酯血糖(TyG)指数、红细胞分布宽度(RDW)和胱抑素C(Cys-C)的关系。方法选择198例CSHF患者作为CSHF组,按纽约心脏病协会(NYHA)心功能分级分为3组:Ⅱ级98例,Ⅲ级58例,Ⅳ级42例;另外选取在本院体检的心功能正常者108例作为对照组。所有入选患者于入院第2天采空腹静脉血测定空腹甘油三酯、血糖、RDW和Cys-C水平,利用空腹甘油三酯和血糖计算TyG指数。结果CSHF组TyG指数明显高于对照组,差异具有统计学意义(P<0.05),两组RDW及Cys-C比较差异无统计学意义(P>0.05);不同心功能分级组间TyG指数比较差异无统计学意义(P>0.05),Ⅳ级组RDW和Cys-C水平明显高于其他两组,差异具有显著统计学意义(P<0.01);Pearson相关性分析显示心功能分级与RDW、Cys-C水平呈显著正相关(P<0.01),与LVEF和LVFS呈负相关(P<0.05),与TyG指数及LVED无相关性(P>0.05);多元回归分析显示LVED、LVEF、LVFS与TyG指数、RDW均无相关性(P>0.05);LVED、LVFS与Cys-C无相关性(P>0.05),LVEF与Cys-C呈独立相关(P<0.05)。结论TyG指数、RDW和Cys-C水平可作为判断CSHF患者病情的指标。Objective To investigate the association of echocardiographic parameters left ventricular enddiastolic diameter(LVED)left ventricular ejection fraction(LVEF)and left ventricular fractional shortening(LVFS),triglyceride glucose(TyG),index,red cell distribution width(RDW)and cystatin C(Cys-C)in patients with chronic systolic heart failure(CSHF).Methods 198 patients with CSHF were selected as CSHF group,and according to New York Heart Association(NYHA)cardiac functional class divided into three groups:98 cases of gradeⅡheart function,58 cases of gradeⅢheart function and 44 cases of gradeⅣheart function.108 cases with normal cardiac function in our hospital were selected as control group.On the second day of admission,fasting venous blood of all patients was collected to measure fasting triglyceride,blood glucose,RDW and Cys-C levels,and TyG index was calculated by fasting triglyceride and blood glucose.Results The TyG index in CSHF group was higher than that of control group,the difference was statistically significant(P<0.05).There was no significant difference in RDW and Cys-C between the two groups(P>0.05).There was no significant difference in TyG index between different concentric functional grading groups(P>0.05).RDW and Cys-C in gradeⅣgroup were significantly higher than those in the other two groups(P<0.01).Pearson correlation analysis showed that cardiac function grading was significantly positively correlated with RDW and Cys-C(P<0.01),negatively correlated with LVEF and LVFS(P<0.05),and had no correlation with TyG index and LVED(P>0.05).Multiple regression analysis showed that LVED,LVEF and LVFS had no correlation with TyG index and RDW(P>0.05);LVED,LVFS were not correlated with Cys-C(P>0.05),while LVEF was independently correlated with Cys-C(P<0.05).Conclusion TyG index,RDW and Cys-C levels can be used as indicators to judge the severity of patients with CSHF.
