摘  要:目的了解西安市某综合医院住院病区、非住院区域环境表面乙肝病毒(HBV)DNA污染状况,为医疗机构环境表面清洁消毒提供科学依据。方法选择西安市某综合医院进行研究,采用实时荧光定量聚合酶链反应(PCR)法对20例临床诊断为乙肝患者居住的住院病区和非住院区域进行环境表面HBV DNA检测。结果住院病区中,临床诊断为乙肝患者居住的环境表面共采样90份,HBV DNA阳性率为11.11%,检出浓度最高为9.902×10^(3) IU/mL,最低为2.763×10^(1) IU/mL;呼叫器按钮和床护栏、床头柜(面和拉手)、水龙头手柄和坐便器按钮、病室和卫生间门把手及开关的HBV DNA阳性率分别为17.39%、13.64%、10.00%、9.09%。非住院区域环境表面共采样279份,HBV DNA阳性率为2.51%;其中,产房、公共区域、检验科及口腔科环境表面HBV DNA阳性率分别为8.82%、6.45%、2.08%、2.08%。非住院区域中,产房3份阳性标本来源于办公用品、隔离产床、产床,HBV DNA浓度分别为5.789×10^(1)、4.288×10^(1)、1.695×10^(2) IU/mL;公共区域2份阳性标本来源于电梯按钮和检验单自助打印机按钮,HBV DNA浓度分别为3.177×10^(1)、5.437×10^(1) IU/mL;口腔科1份阳性标本来源于综合治疗台操作面板,HBV DNA浓度为8.377×10^(2) IU/mL;检验科1份阳性标本来源于标本箱提手,HBV DNA浓度为3.577×10^(1) IU/mL。结论该医院环境表面存在HBV污染,可能存在传播风险;住院病区与非住院区域HBV污染部位不同。Objective To understand the hepatitis B virus(HBV)DNA contamination of environmental surface in inpatient and non-inpatient areas of a general hospital in Xi'an,so as to provide scientific basis for environmental surface cleaning and disinfect of medical institutions.Methods A general hospital in Xi'an was selected for study,the environmental surface HBV DNA was detected by real-time fluorescence quantitative polymerase chain reaction(PCR)in the inpatient and non-inpatient areas of 20 patients with clinical diagnosis of hepatitis B.Results In the inpatient area,90 samples were collected from the environmental surface where the patients with clinical diagnosis of hepatitis B lived;the positive rate HBV DNA was 11.11%,the highest detectable concentration was 9.902×10^(3) IU/mL,and the minimum concentration was 2.763×10^(1) IU/mL;the positive rates of HBV DNA in the pager button and bed guardrail,bedside cabinet(face and handle),faucet handle and toilet button,door handles and switch of ward and toilet were 17.39%,13.64%,10.00% and 9.09%,respectively.A total of 279 samples were collected from the environmental surface of the non-inpatient area,and the positive rate of HBV DNA was 2.51%;the positive rates of HBV DNA on the environmental surfaces of delivery room,public area,laboratory and stomatology departments were 8.82%,6.45%,2.08% and 2.08%,respectively.In the non-inpatient area,three positive samples in the delivery room came from office supplies,isolated delivery bed and delivery bed,and the HBV DNA concentrations were 5.789×10^(1),4.288×10^(1),1.695×10^(2) IU/mL,respectively;two positive samples in the public area came from elevator button and self-service printer button of test sheet,and the HBV DNA concentrations were 3.177×10^(1),5.437×10^(1) IU/mL,respectively;a positive sample in the stomatology department came from the operation panel of the comprehensive treatment table,and the HBV DNA concentration was 8.377×10^(2) IU/mL;a positive sample in the laboratory department came from the sample box ha
