摘　　要：目的分析2013—2019年某三甲医院门诊抗菌药物使用情况。方法调取2013—2019年延安大学附属医院门诊全身用抗菌药物的相关数据,对剂型、用药频度(DDDs)、销售金额及药品销售金额排序/药品DDDs排序(B/A)进行统计分析。结果2013—2019年延安大学附属医院门诊抗菌药物的给药途径以口服为主,占总消耗量比例较稳定;尽管注射制剂在消耗量占比中相对较低,但其销售金额占比几乎均>30%。2013—2019年门诊抗菌药物DDDs以头孢菌素类、大环内酯类为主,其次是硝基咪唑类、青霉素类和氟喹诺酮类,而其他类应用较少。2013—2019年门诊平均每年使用抗菌药物58种;各年累积DDDs至少1次排名前10的药物约占每年总额的75%以上。2013—2019年B/A值出现(或接近)1的品种有头孢丙烯片、克拉霉素缓释片、阿莫西林克拉维酸钾分散片(7∶1)、头孢克肟咀嚼片及头孢克洛干混悬剂。结论延安大学附属医院门诊抗菌药物的使用结构存在一定问题,需要进一步优化。Objective To analyze the antibacterial drug use in outpatient service of a tertiary hospital from 2013 to2019.Methods The data of systemic antibacterial drug used in outpatient service of Yan’an University Affiliated Hospital from 2013 to 2019 was collected,and the dosage form,defined daily dose system(DDDs),sales amount and drug sales amount ranking/drug DDDs ranking(B/A)were statistically analyzed.Results From 2013 to 2019,the main route of administration of antibacterial drug in outpatient service of Yan’an University Affiliated Hospital was oral administration,accounting for a relatively stable proportion of total consumption;although the proportion of injection consumption was relatively low,the proportion of its sales amount was almost higher than 30%.The DDDs of antibacterial drugs of outpatient service in 2013—2019 were mainly cephalosporins and macrolides,followed by nitroimidazoles,penicillin and fluoroquinolones,while other types were less used.A total of 58 kinds of antibacterial drug were used in outpatients on average every year in 2013—2019;the drugs ranking the top 10 in terms of accumulated DDDs at least once a year accounted for more than 75%of the total annual amount.The varieties with B/A value of 1 or close to 1 in 2013—2019included cefprozil tablets,clarithromycin sustained-release tablets,amoxicillin clavulanate potassium dispersible tablets(7∶1),cefixime chewable tablets and cefaclor dry suspension.Conclusion The use structure of antibacterial drug in outpatient service of Yan’an University Affiliated Hospital is problematic,and it needs to be further optimized.
