DEX联合罗哌卡因TAPB在腹腔镜子宫肌瘤剔除术患者中的应用效果及对BCS评分的影响

摘　　要：目的探讨右美托咪定(DEX)联合罗哌卡因腹横肌平面阻滞(TAPB)在腹腔镜子宫肌瘤剔除术中的应用效果。方法选取2020年3月至2021年3月60例于本院接受腹腔镜子宫肌瘤剔除术的患者作为研究对象,按随机数字表法将其分为对照组(罗哌卡因)和观察组(罗哌卡因+DEX),各30例。比较两组的临床效果。结果术后3 d,观察组的40项恢复质量评分量表(QoR-40)评分高于对照组,匹兹堡睡眠质量指数(PSQI)评分低于对照组(P<0.05)。术后4、24 h,两组的视觉模拟评分法(VAS)及舒适度量表(BCS)评分无明显差异(P>0.05);术后6、8、12 h,观察组的VAS评分低于对照组,BCS评分高于对照组(P<0.05)。两组的不良反应总发生率无明显差异(P>0.05)。结论DEX联合罗哌卡因TAPB应用于腹腔镜子宫肌瘤剔除术中,可明显改善患者的炎症反应与应激反应,提高麻醉效果与舒适度,且不会增加药物不良反应,安全性高。Objective To explore the application effect of dexmedetomidine(DEX)combined with ropivacaine transversus abdominis plane block(TAPB)in patients undergoing laparoscopic myomectomy.Methods A total of 60 patients undergoing laparoscopic myomectomy in our hospital from March 2020 to March 2021 were selected as the research objects and divided into control group(ropivacaine)and observation group(ropivacaine+DEX)according to the random number table method,with 30 cases in each group.The clinical effects of the two groups were compared.Results At 3 d after surgery,the 40-item Quality of Recovery Score(QoR-40)score of the observation group was higher than that of the control group,and the Pittsburgh Sleep Quality Index(PSQI)score was lower than that of the control group(P<0.05).At 4 and 24 h after operation,there were no significant differences in Visual Analogue Scale(VAS)and Bruggrmann Comfort Scale(BCS)scores between the two groups(P>0.05);at 6,8 and 12 h after operation,the VAS score of the observation group was lower than that of the control group,and the BCS score was higher than that of the control group(P<0.05).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion DEX combined with ropivacaine TAPB applied in laparoscopic myomectomy can significantly ameliorate the inflammatory response and stress response of patients,improve the anesthetic effect and comfort,without increasing adverse drug reactions,and has high safety.
