脐周八穴埋线对脾虚型多囊卵巢综合征不孕患者的治疗效果及对炎症因子水平的影响

摘　　要：目的探讨脐周八穴埋线对脾虚型多囊卵巢综合征(PCOS)不孕患者的治疗效果及对炎症因子水平的影响。方法选取河北省沧州中西医结合医院2020年5月至2021年6月收治的104例脾虚型PCOS患者作为研究对象,根据随机数表法分为研究组、对照组,各52例。对照组采用中药苍附导痰汤加减治疗,研究组在此基础上联用脐周八穴埋线。记录患者治疗前后脾虚症状评分,检测治疗前后患者血清卵泡刺激素(FSH)、黄体生成素(LH)、雌二醇(E_(2))、空腹胰岛素(FINS)、胰岛素抵抗指数(HOMA-IR)及白介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)水平,随访记录患者月经正常率、排卵率、妊娠率情况。结果治疗后研究组脾虚症状评分及血清LH、FINS、HOMA-IR、IL-6、TNF-α水平均低于治疗前及对照组(P<0.05);血清FSH、E_(2)水平均高于治疗前及对照组(P<0.05)。治疗后研究组月经正常率为90.38%,排卵率为82.69%,均高于对照组的61.54%、51.92%(P<0.05);两组妊娠率比较,差异无统计学意义(P>0.05)。结论脐周八穴埋线可有效改善PCOS患者的脾虚状态,调节激素水平,降低炎症因子水平,并可促进月经及排卵恢复正常。Objective To investigate the therapeutic effect of catgut embedding thread at eight acupoints around umbilicus on infertility patients with polycystic ovary syndrome(PCOS)of spleen deficiency type and its influence on inflammatory factors.Methods A total of 104 patients with PCOS of spleen deficiency type admitted to Hebei Cangzhou Hospital of Integrated Traditional Chinese and Western Medicine from May 2020 to June 2021 were selected as the research objects.According to the random number table method,they were divided into study group and control group,with 52 cases in each group.The control group was treated with Cangfu Daotan Decoction,and the study group was treated with catgut embedding at eight acupoints around umbilicus on this basis.The scores of spleen deficiency symptoms before and after treatment were recorded.The levels of serum follicle stimulating hormone(FSH),luteinizing hormone(LH),estradiol(E_(2)),fasting insulin(FINS),insulin resistance index(HOMA-IR),interleukin-6(IL-6)and tumor necrosis factor-α(TNF-α)were detected before and after treatment.The normal rate of menstruation,ovulation rate and pregnancy rate were recorded.Results After treatment,the scores of spleen deficiency symptoms and the levels of serum LH,FINS,HOMA-IR,IL-6 and TNF-αin the study group were lower than those before treatment and those in the control group(P<0.05).Serum FSH,E_(2) levels were higher than before treatment and control group(P<0.05);the normal menstruation rate of the study group was 90.38%,and the ovulation rate was 82.69%,which were higher than 61.54%and 51.92%of the control group(P<0.05).There was no significant difference in pregnancy rate between the two groups(P>0.05).Conclusions Catgut embedding at eight acupoints around umbilicus can effectively improve the spleen deficiency state of PCOS patients,regulate hormone levels,reduce the level of inflammatory factors,and promote menstruation and ovulation to return to normal.
