延续性护理在阵发性房颤射频消融术后患者随访中的应用价值

摘　　要：目的:探讨延续性护理在阵发性房颤射频消融术后患者随访中的应用价值。方法:纳入2017年1月-2018年6月在雅安市人民医院心内科住院的阵发性房颤射频消融患者89例,使用随机数字的方法将患者分为对照组(n=44)和干预组(n=45),对照组常规护理和门诊随访,干预组延续性护理和门诊随访。比较两组患者术后12个月的临床症状(焦虑、抑郁)及房颤复发率、脑卒中发生率、出血并发症率及华法林达标率是否有差异性。结果:两组患者术后3个月、半年和1年房颤复发率比较,差异均无统计学意义(P>0.05)。干预组SAS和SDS评分均低于对照组,差异有统计学意义(P<0.05)。对照组和干预组脑卒中发生率分别是4.5%和0,差异无统计学意义(P>0.05),对照组和干预组出血并发症发生率分别是15.9%和2.2%,华法林达标率分别是40.9%和84.4%,差异均有统计学意义(P<0.05)。结论:延续性护理可以改善阵发性房颤射频消融患者的焦虑和抑郁,降低出血并发症发生率,提高患者华法林达标率,但是不能降低房颤复发率。Objective:To investigate the value of continuous nursing in follow-up of patients with paroxysmal atrial fibrillation after radiofrequency ablation.Method:A total of 89 patients with paroxysmal atrial fibrillation undergoing radiofrequency ablation in Department of Cardiology,Ya’an People’s Hospital from January 2017 to June 2018 were enrolled.The patients were randomly divided into control group(n=44)and intervention group(n=45).The control group received routine care and outpatient follow-up,while the intervention group received continuous care and outpatient follow-up.The clinical symptoms(anxiety,depression),atrial fibrillation recurrence rate,stroke rate,bleeding complication rate and Warfarin compliance rate were compared between the two groups at 12 months after operation.Result:There were no significant differences in the recurrence rate of atrial fibrillation between the two groups at 3 months,6 months and 1 year after operation(P>0.05).The scores of SAS and SDS in the intervention group were lower than those in the control group,and the differences were statistically significant(P<0.05).The incidence of stroke in the control group and the intervention group was 4.5%and 0,respectively,and the difference was not statistically significant(P>0.05).The incidence of bleeding complications in the control group and the intervention group was 15.9%and 2.2%,and the Warfarin compliance rate was 40.9%and 84.4%,respectively,and the differences were statistically significant(P<0.05).Conclusion:Continuous nursing can improve anxiety and depression in patients with paroxysmal atrial fibrillation undergoing radiofrequency ablation,reduce the incidence of bleeding complications,and improve the Warfarin compliance rate,but it can not reduce the recurrence rate of atrial fibrillation.
