子宫内膜异位症不孕患者腹腔镜手术后应用亮丙瑞林、曲普瑞林的效果

摘　　要：目的:分析子宫内膜异位症不孕患者腹腔镜手术后应用亮丙瑞林、曲普瑞林的效果。方法:选取2018年4月-2019年4月英山县人民医院行腹腔镜手术治疗的子宫内膜异位症不孕患者144例。按照随机数字表法将其分为亮丙瑞林组和曲普瑞林组,各72例。两组均行腹腔镜手术治疗。亮丙瑞林组术后给予注射用醋酸亮丙瑞林微球治疗,曲普瑞林组术后给予注射用醋酸曲普瑞林治疗。比较两组治疗前后卵巢储备功能、可溶性血管内皮生长因子-1(sFlt-1)、单核细胞趋化蛋白-1(MCP-1)、胰岛素样生长因子-Ⅰ(IGF-Ⅰ)、基质金属蛋白酶-9(MMP-9)、抗子宫内膜抗体(EMAb)、癌抗原125(CA125)、糖链抗原199(CA199)水平及临床疗效、不良反应、妊娠率、复发率。结果:治疗后,亮丙瑞林组FSH、LH水平均高于曲普瑞林组,E_(2)、P水平均低于曲普瑞林组(P<0.05)。治疗后,亮丙瑞林组sFlt-1水平高于曲普瑞林组,MCP-1、IGF-1、MMP-9、EMAb水平均低于曲普瑞林组(P<0.05)。治疗后,亮丙瑞林组CA125、CA199水平均低于曲普瑞林组(P<0.05)。亮丙瑞林组总有效率高于曲普瑞林组(P<0.05)。亮丙瑞林组不良反应发生率低于曲普瑞林组(P<0.05)。亮丙瑞林组妊娠率高于曲普瑞林组,子宫内膜异位症复发率低于曲普瑞林组(P<0.05)。结论:与曲普瑞林相比,子宫内膜异位症不孕患者腹腔镜手术后应用亮丙瑞林效果理想,可显著改善患者卵巢储备功能,抑制CA125、CA199异常表达,提高妊娠率,降低复发率,安全性高。Objective:To analyze the effect of the application of Leuprorelin and Triptorelin in infertility patients with endometriosis after laparoscopic operation.Method:A total of 144 infertility patients with endometriosis who underwent laparoscopic operation treatment in Yingshan County People’s Hospital from April 2018 to April 2019 were selected.They were divided into Leuprorelin group and Triptorelin group according to the method of random number table,72 cases in each group.Laparoscopic operation was performed in both groups.The Leuprorelin group was given the Leuprorelin Acetate Microspheres for Injection treatment after operation,and the Triptorelin group was given the Triptorelin Acetate for Injection treatment after operation.The ovarian reserve function,the levels of soluble fms-like tyrosine kinase-1 (sFlt-1),monocyte chemoattractant protein-1 (MCP-1),insulin-like growth factor-Ⅰ (IGF-Ⅰ),matrix metalloproteinase-9 (MMP-9),anti-endometria antibody (EMAb),cancer antigen 125 (CA125),carbohydrate antigen 199 (CA199) before and after treatment,clinical efficacy,adverse reactions,pregnancy rate and recurrence rate were compared between the two groups.Result:After treatment,FSH and LH levels were higher in the Leuprorelin group than in the Triptorelin group,and Eand P levels were lower than in the Triptorelin group (P<0.05).After treatment,sFlt-1 level was higher in the Leuprorelin group than in the Triptorelin group,and the levels of MCP-1,IGF-1,MMP-9,and EMAb were all lower than in the Triptorelin group (P<0.05).After treatment,the levels of CA125 and CA199 were lower in the Leuprorelin group than in the Triptorelin group (P<0.05).The total effective rate in the Leuprorelin group was higher than that in the Triptorelin group (P<0.05).The incidence of adverse effects in the Leuprorelin group was lower than in the Triptorelin group (P<0.05).The pregnancy rate was higher in the Leuprorelin group than in the Triptorelin group,and the endometriosis recurrence rate was lower than in the Triptorelin group (P<0.05).
