糖皮质激素联合醋酸戈舍瑞林缓释植入剂对子宫内膜异位症腹腔镜术后患者的影响

摘　　要：目的:探究糖皮质激素联合醋酸戈舍瑞林缓释植入剂对子宫内膜异位症腹腔镜术后患者的影响。方法:选取2018年2月-2019年2月深圳市龙岗区人民医院收治的100例子宫内膜异位症腹腔镜术后患者,采用随机数字表法分为观察组和对照组,各50例。对照组采用醋酸戈舍瑞林缓释植入剂治疗,观察组在对照组基础上联用糖皮质激素治疗。评估两组术后妊娠率、复发率、激素水平、疗效满意度。结果:观察组术后妊娠率为96.00%,高于对照组的84.00%(P<0.05);观察组复发率为2.00%,低于对照组的14.00%(P<0.05);治疗3个月后,两组血清黄体生成素(LH)、卵泡雌激素(FSH)、雌二醇(E)、血管内皮生长因子(VEGF)水平较治疗前均降低,且观察组均低于对照组(P<0.05);观察组疗效满意度为98.00%,高于对照组的86.00%(P<0.05)。结论:糖皮质激素联合醋酸戈舍瑞林缓释植入剂治疗子宫内膜异位症腹腔镜术后患者能有效提高术后妊娠率,降低复发率,降低性激素及血管内皮生长因子水平,提高疗效满意度。Objective:To explore the effect of glucocorticoid combined with Goserelin Acetate Sustained-release Implant on patients with endometriosis after laparoscopic surgery.Method:A total of 100 patients with endometriosis after laparoscopic surgery who were admitted to Longgang District People’s Hospital of Shenzhen City from February 2018 to February 2019 were selected and divided into an observation group and a control group by random number table method,with 50 cases in each group.The control group was treated with Goserelin Acetate Sustained-release Implant,and the observation group was treated with glucocorticoid on the basis of the control group.The postoperative pregnancy rate,recurrence rate,hormone level and efficacy satisfaction of the two groups were evaluated.Result:The postoperative pregnancy rate of the observation group was 96.00%,which was higher than 84.00% of the control group (P<0.05).The recurrence rate of the observation group was 2.00%,which was lower than 14.00% of the control group (P<0.05).After 3 months of treatment,the levels of serum luteinizing hormone (LH),follicle estrogen (FSH),estradiol (E) and vascular endothelial growth factor (VEGF) in the two groups were lower than those before treatment,and those in the observation group were lower than those in the control group (P<0.05).The efficacy satisfaction of the observation group was 98.00%,which was higher than 86.00% of the control group (P<0.05).Conclusion:Glucocorticoid combined with Goserelin Acetate Sustained-release Implant in the treatment of endometriosis after laparoscopic surgery can effectively improve the postoperative pregnancy rate,reduce the recurrence rate,reduce the levels of sex hormones and vascular endothelial growth factor,and improve the efficacy satisfaction.
