摘  要:目的通过基于气相色谱飞行时间质谱(GC-TOF/MS)的非靶向代谢组学方法探讨羊水中室间隔缺损(VSD)自然闭合差异代谢物。方法选取2018年5月至9月在南京医科大学附属妇产医院临床遗传实验室进行产前诊断的羊水样本14例,其中产前胎儿超声心动图检测存在VSD且产后随访VSD自然闭合胎儿母体羊水样本5例,对照组为产前诊断为VSD但产后随访未自然闭合胎儿母体羊水样本9例,采用GC-TOF/MS技术检测所有样本的差异代谢物。通过主成分分析(PCA)和正交偏最小二乘法判别分析(OPLS-DA)比较VSD自然闭合组和未自然闭合组代谢谱差异,通过多维和单维统计分析寻找差异代谢物。结果PCA和OPLS-DA模型均显示VSD自然闭合组和未自然闭合组间无明显分离趋势。单维和多维分析均显示鼠李糖和苯甲酸在VSD自然闭合组较未自然闭合组显著降低。结论本研究发现了VSD自然闭合差异代谢物,为预测VSD能否自然闭合提供了潜在的标志物,也为VSD自然闭合病理机制的探索提供更多线索。Objective To explore differential metabolites in amniotic fluid for spontaneous closure of ventricular septal defect(VSD)by an untargeted metabolomics method based on gas chromatography time-of-flight mass spectrometry(GC-TOF/MS).Methods A total of 14 residual amniotic fluid samples for prenatal diagnosis were collected from the Clinical Genetics Laboratory of our hospital from May to September 2018.Among them,5 samples were obtained from the maternal amniotic fluid in which the fetuses with VSD were detected by prenatal fetal echocardiography and spontaneous closures were found during postpartum follow-up,while 9 samples from the maternal amniotic fluid,in which the fetuses were diagnosed with VSD in prenatal examination but their ventricular septal defect unclosed spontaneously during postpartum follow-up,were considered as controls.All samples were detected by GC-TOF/MS.The principal component analysis(PCA)and orthogonal partial least square discriminant analysis(OPLS-DA)were applied to compare the differences of metabolic profiles between the spontaneous closure group and the unclosed group of VSD,and the differential metabolites were found by multivariate and univariate statistical analysis.Results Both PCA and OPLS-DA models showed that there were no significant separation trend between the spontaneous closure group and the unspontaneous closure group of VSD.Both univariate and multivariate statistics analysis showed that the levels of rhamnose and benzoic acid in the spontaneous closure group were significantly lower than those in the unspontaneous closure group.Conclusion The differential metabolites of spontaneous VSD closure were found in this study,which may provide potential markers for predicting spontaneous closure of VSD,and also provide more clues for exploring the pathological mechanism of spontaneous closure of VSD.
